THIS REQUEST FOR INFORMATION (RFI) IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES, AND OTHER MARKET INFORMATION IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH (IAW) FAR 15.201(e). DISCLAIMER: This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. IAW FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a sources sought/RFI only. The purpose of this sources sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 339113. Responses to this sources sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 5 Network Contracting Office, is seeking sources that can provide equipment for Bed Rentals for the VAMHCS - 10 N Greene St, Baltimore, MD 21201 STATEMENT OF WORK 1. Introduction The purpose of this Statement of Work (SOW) is to outline the tasks and responsibilities of the vendor in supplying Rental Services for specialized beds and overlays (mattresses, etc.) for Department of Veterans Affairs, Maryland Health Care System (VAMHCS) located at 10 N. Greene Street, Baltimore, MD and the Perry Point Health Care located at Avenue D, Perry Point, Maryland and Loch Raven Health Care located at Almeda Drive, Loch Raven, Maryland. 2. Scope The vendor shall provide the following services at VAMHCS on a continuous basis for the duration of this agreement from October 1, 2023 to September 30, 2024. Delivery and pickup of requested rental products to and from specified VAMHCS locations within 24 hours of delivery request for rental beds as follows: Triflex 2 bed platform Mattress/overlay and accessories Bariatric Expandable Bed Mattress/overlay and accessories Oncare Harmony Hi Lo Bed Mattress/overlay and accessories Spirit Hi Lo, Hi Lo Plus, and Select Hi Lo Mattress/overlay and accessories Free Standing Trapeze Bariatric Hi Lo Bed Mattress/overlay and accessories H Frame-PT. Helper Sentinel II Bed Mattress/overlay and accessories Bari 10-a Bariatric Bed Mattress/overlay and accessories Hercules unit 3. Implementation Products are continuing to be requested for delivery/pick-up by nurses for patient care from current vendor. 4. Period of Performance/Acceptance The period of performance is 10/1/2023 September 30, 2024. Acceptance of service shall be made manifest through certification of monthly invoices/inventory reports for which invoiced services were delivered. Deliverables include, in addition to the Product and services outlined in the Scope above, monthly invoices which include a weekly inventory that reflects Product movement over the preceding week. 5. Vendor Responsibilities These services require performance, for Product delivery, on VAMHCS facilities. The Contractor is responsible for providing the deliverables indicated above for Acceptance of services. Problems/unforeseen situations will be addressed through implementation of conference calls, face-to-face meetings, and formal correspondence, as deemed appropriate for the given circumstances. The Contractor shall identify three to five employees to be cleared to receive VAMHCS identification badges as Contractor Drivers with authority to deliver and pick up VAMHCS product as called for under the terms of this Agreement. 6. Government Responsibilities VAMHCS shall provide shall request both deliveries and pick-ups of Product. VAMHCS shall validate accomplishment of the requested action. 7. Hours of Operation The Department is open 24/7, which the vendor may be contacted anytime. In accordance with 5 U.S.C. 6103, Executive Order 11582 and Public Law 94-97 the following national holidays are observed and for the purpose of this contract are defined as Legal Federal official holidays Columbus Day Second Monday in October Veteran s Day November 11th Thanksgiving Day Fourth Thursday in November Christmas Day December 25th When a holiday falls on a Sunday, the following Monday will be observed as a National Holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a National Holiday by U.S. Government Agencies. 8. Travel All travel shall be responsibility of the vendor.
