Statement of Work TopCon NW 500 Non-Mydriatic Retinal Camera INTRODUCTION: The VA Boston Healthcare System Surgical Department, Ophthalmology Section, located at the Jamaica Plain Campus, is requesting a replacement TopCon NW 500 Retinal Camera equipment. This camera will replace our current fundus camera (also Topcon), which may be approaching 20+ years old (in fact, the current camera operates on Windows 7 and reportedly is no longer supported). Fundus photography is essential for capturing high-resolution optic nerve photos and macular images for monitoring progression of diseases like glaucoma, diabetic retinopathy, macular degeneration. In the field of uveitis, fundus imaging is indispensable for monitoring progression of infectious retinitis or autoimmune choroidal or retinal inflammation. Due to continued use and need of this equipment, Ophthalmology needs this in place for patient care. This machine will need to be connected to the VA network and the VA will retain all information. Mark Crowe, who is the contractor representative, will be on site after delivery has been made for installation purposes only. He will be onsite to unpack the boxes, inspect the parts, and to set-up the equipment. Mr. Crowe will be sure the equipment is working properly before staff begin to use it on patients and will answer any questions that staff may have on the updated equipment. Vendor has access to local admin accounts since they know the proprietary application and will assist BioMed in troubleshooting if needed. Once the system is installed, the equipment is maintained by BioMed in house. If any issues arise with the equipment, the contractor representative will provide guidance to BioMed on how to mitigate those issues via phone/email. The vendor doesn t need any elevated privileges to complete the necessary work needed. TopCon, works closely with the contractor representative, Mark Crowe. TopCon will be providing training to staff due to the upgraded technology. Staff will be trained and will be able to ask questions as needed during this time. The Section Chief of Ophthalmology (Dr. Nicholas Butler) will coordinate a date/time with a TopCon representative for the necessary training. The contractor representative will be accompanied by the Clinical Engineering during the installation. All contractor personnel shall report and obtain a short-term identification badge issued by the COR or government designated representative. Such badge shall be worn by the individual and prominently displayed at all times while on VA Property. No employee of the contractor shall enter the site without a valid identification badge issued by the VA. In order to obtain a short-term identification badge, contractor personnel shall present to the COR a valid (non-expired) photo identification issued by a US federal, state, or local government agency. After hours all contractor personnel (without exception) must report to the Clinical Engineering Department, to sign in where a contractor identification badge is verified before any work is performed. Contractor personnel will be escorted by Clinical Engineering staff for the duration of work performed and return to the same location to sign out and turn-in documented service report for the service performed.
