Amendment 1 - Small Business set-aside has been removed. This requirement is now Full & Open competition. Additionally, the requirement has been extended through Tuesday, September 26, 8 AM PDT. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.� � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � �� Solicitation # RFQ-23-PHX-084 and this notice is issued as a Request for Quotation (RFQ).� This is a combined synopsis/solicitation for commercial commodities/services. Submit written quotes only, oral offers will not be accepted. All firms or individuals responding must be registered with the System for Award Management (SAM). This requirement is under North American Industrial Classification Standard (NAICS) 325412 Pharmaceutical Preparation Manufacturing; Small Business Size Standard: 1,300 employees. � REQUIREMENTS:� Indian Health Service (IHS) � Phoenix Area Office (PAO), 40 North Central Ave., Phoenix, Arizona 85004-4424 has a requirement for Brand Name or Equal IV solutions and supplies, which play a vital part of patient wellness. The Government intends to enter into a single Blanket Purchase Agreements (BPA), in accordance with FAR 13.303. A blanket purchase agreement (BPA) is a simplified method of filling anticipated repetitive needs for supplies or services by establishing ""charge accounts"" with qualified sources of supply. A Blanket Purchase Agreement (BPA) does not have any funds committed; it provides a framework for quick Calls to be completed, providing funds availability. The term for this BPA is five years. During that five years, the Gov�t can issue Calls against the contract to fund purchase of the items identified in the pricing schedule. Calls are a lump-sum of money used in a declining balance fashion, to fund repetitive supply purchases, as provided under the BPA framework (e.g. pricing schedule and terms and conditions). Calls will not be competed. This procurement is for NEW Equipment ONLY; no remanufactured or ""gray market"" items. Vendor shall be an Original Equipment Manufacturer (OEM authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system, such that OEM warranty and service are provided and maintained by the OEM). All, warranty and service associated with the equipment shall be in accordance with the OEM terms and conditions. All Equipment must be covered by the manufacturer's warranty. The quote MUST include a copy of the authorized distributor letter from the manufacturer to verify that the vendor is an authorized distributor of the products being quoted; failure to provide evidence of this may result in your proposal not being further considered. ***QUESTIONS DUE DATE: 09/21/2023 10:00 AM PDT (AZT) � all questions must be submitted via email. This is an open-market combined synopsis/solicitation for products as defined herein. The government intends to perform a comparative evaluation of offers, IAW FAR 13.106-2(b)(3), to award a blanket purchase agreement (BPA), as a result of this combined synopsis/solicitation, to the responsive and responsible vendor, whose offer represents the best value to the Gov�t using price and other factors. This combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAR) FAC 2023-05 September 7, 2023.
