- Residential Housing and Health Care for Homeless Veterans (HCHV) Contract Residential Shelter (CRS) Medical Respite. Overton Brooks VA Medical Center Shreveport, Louisiana DESCRIPTION: This is a Sources Sought to determine the availability of potential sources having the skills and capabilities necessary to provide the services as part of its community-based Health Care for Homeless Veterans (HCHV) program. The HCHV program s Contract Emergency Residential Services (CRS) programs target and prioritize Veterans experiencing homeless who are transitioning from literal street homelessness, being discharged from institutions, including those in need of medical respite, and who recently became homeless and require safe and stable living arrangements while they seek permanent housing. BACKGROUND: Through the HCHV program, VA provides supportive services to Veterans and facilitates their access to a broad range of medical, mental health, and rehabilitative services. The purpose of this request for information/Source Sought is to determine the availability of potential sources in the surrounding areas within 25 miles of the Shreveport VA s Overton Brooks Veterans Affairs Medical Center (OBVAMC) at 510 E Stoner Ave, Shreveport, LA 71101 that can provide temporary housing and supportive services in a community-based setting to Veterans with special needs who are experiencing homelessness. This notice is searching for quality Medical Recuperative Care (RC) and housing services for the most vulnerable and at-risk homeless Veterans and provide a safe, healthy, and secure setting to promote their rehabilitation goals and discharge to permanent housing. Services will be provided to VHA healthcare-eligible Veterans who may be discharging from the VA Medical Center and other medical facilities for discharged or planned medical procedures, requiring stabilization prior to and after hospitalization. The contractor will be required to provide a low-barrier therapeutic and rehabilitative milieu and attendant services to address medical rehabilitation, while targeting the underlying factors contributing to homelessness. The contractor will not be required to provide detoxification or other acute level treatment. SERVICES TO BE PROVIDED General Responsibilities: The contractor will provide sufficient facilities to accommodate 5 Veterans with residential placement which will involve: Registered Nurse & Licensed Practical Nurse on-site 24/7 to provide all medical related care needs and interventions. Contractor is to follow Louisiana State RN & LPN board guidelines for hiring and staffing. 24 hours/7 days per week medication management. Case management services on-site for no less than 10 hours per week. Additional hours may be necessary, commensurate with the needs of each Veteran. Mental Health Services provider available for assessment and provision of interventions, referrals, and monitoring of mental health conditions, as clinically indicated. 24/7 residential staffing on-site. Room and Board: The contractor is required to provide sufficient facilities to accommodate 5 Veteran residents free of charge and shall be accessible to the Veterans 7 days a week and 24 hours per day. Contractor will arrange On-call medical provider (MD/PA/NP) coverage for orders between 4 pm and 8 am Monday through Friday. All day on Saturdays and Sundays. Contractor must also solidify back-up medical provider coverage on dates that HPACT lead provider is on vacation, federal holidays, or on unplanned sick leave. Additional miscellaneous staffing structure for meeting this level of care, (i.e., nutrition, security, facilities, etc.) must be submitted with offers. THIS IS NOT A SOLICITATION ANNOUNCEMENT. This is a Sources Sought synopsis. Responses shall be submitted by email to thomas.kovarovic@va.gov. Provide the requested information below. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classifications (Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), Hub zone, 8(a), small, small disadvantaged, woman owned small business, FSS/GSA contract schedule holders or large business) relative to NAICS 623220 Residential Mental Health and Substance Abuse Facilities (size standard $19M). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this source sought synopsis, a solicitation announcement may be published on the SAM Contract Opportunities website. Responses to this source sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this Sources Sought announcement.
