- This is a PRE-SOLICITATION SOURCES SOUGHT NOTICE only. No proposals are being requested or accepted with this notice. THIS IS NOT A SOLICITATION FOR PROPOSALS OR PRICING AND NO CONTRACT SHALL BE AWARDED FROM THIS NOTICE. This notice shall not be construed as a solicitation or as an obligation on the part of the Indian Health Service. Responses to this notice will be treated only as information for the Government to consider as part of their market research efforts. The information provided will be used by the Government in developing its acquisition strategy regarding possible set aside for Indian-Owned Small Business Economic Enterprise (ISBEE) and other socio-economic categories of small business. Interested parties are responsible for adequately marking proprietary, restricted or competition sensitive information contained in their response. The Government does not intend to pay for the information submitted in response to this notice and parties responding will not be entitled to payment for direct or indirect costs incurred in responding to this notice. The North American Industry Classification System (NAICS) code for this requirement is 811210, Electronic and Precision Equipment Repair and Maintenance, with an SBA Small Business Size Standard of $34.0 million. 1. Please identify any of your firm�s existing contract vehicles (e.g., GSA federal supply schedules) which provide similar services as those described in the Statement of Work (attached). 2. Please provide all social-economic categories of your firm (e.g., ISBEE, SDVOSB, VOSB, WOSB, IEE, Large Business, 8a, etc.) as well as your firm�s SAM Unique Entity ID number (replaces DUNS number) and Government Contract POC. STATEMENT OF WORK PROJECT TITLE:�� Sterilizer and Washer Preventative Maintenance Service for Fort Peck Service Unit (FPSU) BACKGROUND/PURPOSE: Preventative Maintenance services on a quarterly basis are required on sterilizing equipment and components per The Joint Commission requirements.� SCOPE: Quarterly Preventative maintenance is required for the service unit�s medical sterilizers, boilers, RO Systems and semi-annual maintenance on the washer disinfector systems equipment located at the Fort Peck Service Unit�s medical clinics, Verne E Gibbs Health Center, White Buffalo Clinic located in Poplar, MT, and Chief Redstone Health Center located in Wolf Point, MT. The preventative maintenance will be completed per manufacturer�s recommendation for annual maintenance. APPLICABLE DOCUMENTS:� Joint Commission Accreditation, Manufacturer Specifications for Equipment SPECIFIC TASKS: Provide preventative maintenance services.� Schedule must be coordinated through the FPSU POC.� Task 1: Qualified technician shall conduct complete preventative maintenance per manufacturer�s recommendation for the FPSU sterilizer and washer equipment.� The Contractor shall provide all transportation, personnel, materials, labor, and equipment required to perform the preventative maintenance service.��Task 2:� Provide Written Report that Documents Location, Barcode, Serial Number, Manufacturer, Nomenclature, Model number, PM checklist with results, any equipment issues/comments or required parts.� Documentation listing: for each piece of equipment will have documentation which reflects Location, Barcode, Serial Number, Manufacturer, Nomenclature, Model number, PM checklist with results, any equipment issues/comments or required parts.� DELIVERABLES AND DELIVERY SCHEDULE: Biomedical preventative maintenance shall be provided within 30 calendar days of award.� A written report of the results shall be provided by email to the Fort Peck Service Unit Facility Manager within 15 calendar days of completion.� GOVERNMENT-FURNISHED EQUIPMENT AND INFORMATION:� FPSU will present all sterilizers and washers to the Contractor for preventative maintenance services.� �PLACE OF PERFORMANCE: Getinge 433-HC-E sterilizer, boiler, RO System and washer disinfector components are located at the Fort Peck Service Unit�s medical clinic, Verne E Gibbs Health Center, White Buffalo Clinic located in Poplar, MT and Chief Redstone Health Center located in Wolf Point, MT.� PERIOD OF PERFORMANCE:�� Anticipated start date is 30 days after contract award for 12 months.� SECURITY: Contractor technicians are required to check in with FPSU POC upon arrival at facility to obtain access and location information.
