- The reason for Amendment 0007 is to make changes to the solicitation and to answer questions. No additional questions will be answered. The response date for receipt of proposals is NOT extended; it remains as September 28, 2022, 12:00 PM Mountain Time. a. Section B.2 Schedule of Services is changed as follows: B.2 SCHEDULE OF SERVICES Contract Minimum: $590,000.00 Contract Maximum: $100,000.00 b. 52.216-19 ORDER LIMITATIONS (OCT 1995) (a) Minimum order. When the Government requires supplies or services covered by this contract in an amount of less than $590,000.00 the Government is not obligated to purchase, nor is the Contractor obligated to furnish, those supplies or services under the contract. (b) Maximum order. The Contractor is not obligated to honor (1) Any order for a single item in excess of $13,000,000; (2) Any order for a combination of items in excess of $100,000,000; or (3) A series of orders from the same ordering office within 30 days that together call for quantities exceeding the limitation in paragraph (b)(1) or (2) of this section. (c) If this is a requirements contract (i.e., includes the Requirements clause at subsection 52.216-21 of the Federal Acquisition Regulation (FAR)), the Government is not required to order a part of any one requirement from the Contractor if that requirement exceeds the maximum-order limitations in paragraph (b) of this section. (d) Notwithstanding paragraphs (b) and (c) of this section, the Contractor shall honor any order exceeding the maximum order limitations in paragraph (b), unless that order (or orders) is returned to the ordering office within 10 days after issuance, with written notice stating the Contractor's intent not to ship the item (or items) called for and the reasons. Upon receiving this notice, the Government may acquire the supplies or services from another source. (End of Clause) c. Questions and answers are as follows: 1. The answer to question 20 states, ""One counseling patient care room per 1000 projected enrollees is required for VA mental health staff, and one consult patient care room per 1000 projected enrollees is required for VA support staff..."" Could the VA describe the difference between counseling patient care rooms and consult patient care rooms? What is the minimum square footage for a counseling patient care room? Answer to Question 1: Please refer to PG 18-9 Space Planning Criteria (March 1, 2022) for this information http://www.cfm.va.gov/til/space/spChapter265.pdf 2. Based on the answers to questions 20, 43, 44, and 45 we wish to confirm our understanding of the patient care area requirements for this CBOC. We believe the minimum requirements for exam rooms, consult rooms, and counseling rooms based on current enrollment to be as follows: -27 primary care exam rooms -2 women's health/procedure exam rooms -10 counseling patient care rooms -10 consult patient care rooms Could the VA confirm that these rooms are required for the Denton CBOC? Answer to Question 2: Based on current enrollees at the Denton CBOC, the above room count is correct. However, additional rooms will be needed over time to support the projected enrollee growth over the life of the contract. In amendment 5, question 45, the VA notes that one one counseling patient care room for VA Mental Health Providers per projected 1,000 enrollees is required. Please confirm that the counseling patient care room for VA Mental Health Providers will also serve as the office for VA staffed mental health providers. If so: can these rooms be integrated into the PACT zones along side patient exam room is one door acceptable from the hallway and no door to the PACT work zone (for privacy) Answer to Question 3: Yes, the counselling patient care rooms will be staffed by VA mental health providers. Can these rooms be integrated into the PACT zones along side patient exam room Please refer to the Community Based Outpatient Clinics Prototype for Standardized Design and Construction of Community Based Outpatient Clinics at 04_PLANNING COMPONENTS + MODULES_CBOC_Final.indd (va.gov) as referenced in the PWS. The mental health consult rooms are to be adjacent to the PACT zone as noted. is one door acceptable from the hallway and no door to the PACT work zone (for privacy) Yes, one door from the hallway is acceptable for counseling rooms. VA standards are to provide two social workers per primary care PACT team. The VA has said they will provide two social workers. Please confirm that this is correct. Answer to Question 4: Yes, the VA will provide the PACT social workers. Please confirm that to date, amendments for this proposal have not included an SF-30 form and that the offeror simply needs to acknowledge they have seen each amendment in the proposal. (In the past, we have traditionally signed the SF-30 and provided a copy in the proposal). Answer to Question 5: Correct, amendments should be acknowledged on page 7 of the solicitation section titled ACKNOWLEDGEMENT OF AMENDMENTS. Would the government like resumes or CV info on any candidates other than providers? Answer to Question 6: No. While we note there have been a number of Amendments to the Solicitation, there have not been any SF30 s provided on which to acknowledge the Amendments. Please advise as to how you would like to see the Amendments acknowledged. Answer to Question 7: Amendments should be acknowledged on page 7 of the solicitation section titled ACKNOWLEDGEMENT OF AMENDMENTS. In Amendment #5, Questions & Answers number 16 and 33, the VA states 180 days from contract award to commencement of the provision of care as stated in 11.2.2. However, in the original RFP on page 105 paragraph 11.2.2 states 120 days from contract award and the new revised Performance Work Statement (in Amendment #5) on page 84 of 100, paragraph #19.1.2 it was changed from 120 to a 90 day start up. Will the VA please confirm that the start-up time frame is 180 calendar days from contract award? Answer to Question 8: Yes, the start-up time frame for contract award to the clinic opening to see patients is 180 calendar days. d. All other terms and conditions remain unchanged.
