- This is a SOURCES SOUGHT NOTICE only. No proposals are being requested or accepted with this notice. THIS IS NOT A SOLICITATION FOR PROPOSALS OR PRICING AND NO CONTRACT SHALL BE AWARDED FROM THIS NOTICE. This notice shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs. Responses to this notice will be treated only as information for the Government to consider as part of their market research efforts. The information provided will be used by the Government in developing its acquisition strategy regarding possible set aside for Veteran Owned and other socio-economic categories of small business. Interested parties are responsible for adequately marking proprietary, restricted or competition sensitive information contained in their response. The Government does not intend to pay for the information submitted in response to this notice and parties responding will not be entitled to payment for direct or indirect costs incurred in responding to this notice. The North American Industry Classification System (NAICS) code for this requirement is 484210, Used Household and Office Goods Moving, with an SBA Small Business Size Standard of $34 million. Please identify any of your firm s existing contract vehicles (e.g., GSA federal supply schedules) which provide similar services as those described in the Statement of Work (attached). Please provide all social-economic categories of your firm (e.g., SDVOSB, VOSB, WOSB, Large Business, 8a, etc.) as well as your firm s SAM Unique Entity ID number (replaces DUNS number) and Government Contract POC. See attached draft Statement of Work below for details of the requirement. STATEMENT OF WORK Minneapolis Veteran Affairs Medical Center Moving Services Introduction: The Minneapolis Veteran Affairs Medical Center (MPLS VAMC) has a requirement to move office furniture and related items on a continuous basis. The Contractor will need to provide a wide variety of moving services throughout the Main Hospital and Community Based Outlying Clinics (CBOC). This service is crucial to the operation of the Minneapolis VA. Objectives: Establish a moving services contract with a reputable contractor that can provide a full range of moving services. Statement of Work: This Moving Services contract will encompass all facilities that fall under the Minneapolis VA. See list below: Building 70 VA Minneapolis Medical Center (One Veterans Drive, Minneapolis, MN) Building 9 5447 Minnehaha Avenue South, Minneapolis, MN Building 10 5445 Minnehaha Avenue South, Minneapolis, MN Building 49 One Veterans Drive, Minneapolis, MN Building 68 One Veterans Drive, Minneapolis, MN Building 76 Spinal Cord Injury Fisher House 1 (One Veterans Drive, Minneapolis, MN) Building 77 Modular Building Fisher House 1 (One Veterans Drive, Minneapolis, MN) Building 79 Specialty Clinic Building 89 Fort Snelling, Minnehaha Ave Building 217 Fort Snelling, Minnehaha Ave Building 222 Fort Snelling, Minnehaha Ave Building 224 Fort Snelling, Minnehaha Ave CBOC s in Minnesota Albert Lea, Ely, Hibbing, Mankato, Maplewood, Ramsey, Rochester, St. James, and Shakopee CBOC s in Wisconsin Chippewa Falls, Hayward, Rice Lake, and Twin Ports. * Locations may be added or deleted based on the VA s mission. The contractor will be responsible for all aspects of moving office furniture and related items. Contractor will be responsible for the assembly, disassembly, installation, reconfiguration, and transportation of government-owned furnishings. Items to be moved include, but are not limited to, the following: Desks File cabinets Furniture Bookcases Chairs/benches Miscellaneous medical equipment (beds, exam tables, etc.) Boxes and/or moving bins/totes (totes provided by MPLS VAMC) Contractor is responsible for packaging and protecting all government-owned furniture and furnishings. Contractor shall wrap, box up, load, transport, and deliver identified items from the current location to a new location specified by the VA Contracting Officer Representative/Point of Contact (COR/POC). Contractor will deliver items to the designated floor, and room number listed on each box. Contractor may also be required to rotate items in and out of storage or transport miscellaneous items to and from other locations (i.e. local recycling center). Contractor shall provide all labor, supervision, equipment, tools, vehicles, supplies, and materials necessary to perform all tasks related to moving furniture and supplies. Contractor shall furnish all packing supplies and equipment to include, but not limited to: Boxes, tape, and wrapping (bubble and shrink-wrap) Furniture pads and blankets Hand trucks and furniture dollies Hand and power tools needed to disassemble and reassemble office furniture/equipment *Note: The VA will provide plastic moving totes. Contractor will be responsible to move the totes. Vehicles used to transport government property must be leased, rented, or owned by the contractor. Personally-owned vehicles and trailers are prohibited. The government is not responsible for damage or loss of contractor s tools or equipment. The Contractor shall designate an on-site supervisor. The designated supervisor will be the point of contact to coordinate each moving service activity with the VA COR/POC and/or designated VA staff (Move Coordinator/Interior Design Coordinator). Additionally, the designated supervisor will be responsible for directing the movement of crew members. The Contractor shall provide sufficient personnel/crews based on workload. In addition, the Contractor shall ensure all crew members are physically able to perform all duties required by this contract. Employees must be able to lift up to 50 pounds repetitively and occasionally 70 pounds unassisted. Contractor will provide a minimum of two personnel when performing services. The Contractor employees attire will display the contractors name or logo on their shirt that clearly identifies them as a representative of their company. Attire shall always present a professional appearance. Attire will include long pants, shirts with sleeves, and appropriate closed-toed foot gear. Contractor s office must be within 60 miles of the MPLS VA Main Hospital (Bldg 70). Contractor must have five years or more experience moving office furniture and related items. Property Damage: Contractor shall take necessary precautions to avoid damaging walls, floors, elevators, and elevator lobbies when performing move activities. Wall and floor protection will be used to prevent damages. Contractor must maintain insurance that covers damage to property. Contractor will be responsible for all costs associated with damages related to move actives. Working hours: Anticipated hours of work will be Monday through Friday from 7:00 am 5:00 pm. Additional hours must be coordinated through the COR/POC. Due to the changing dynamics of the VA, the Contractor may be required to perform service after hours, on weekends, or over holidays. Some projects may only be performed during non-duty hours to avoid disruptions to patient care. Overtime work must be first approved and assigned by the COR/POC. When this occurs, overtime will be paid at 1.5 times the hourly rate. Observed Federal Holidays: New Year s Day Martin Luther King Birthday Washington s Birthday Memorial Day Juneteenth Independence Day Labor Day Columbus Day Veteran s Day Thanksgiving Day Christmas Day Safety: Contractor is responsible for maintaining a safe working environment for their employees as well as VA patients, visitors, and staff. Appropriate Personnel Protective Equipment will be worn while performing work at the MPLS VA. Work areas will be kept clean throughout the day and all trash and demolished materials will be properly disposed of daily. Tools and equipment will not be left unattended in areas open to the public. Security: Contractor s personnel will utilize a temporary vendor s security badge until a permanent security PIV badge can be issued. Temporary badge use will not exceed 14 calendar days. The Contractor shall comply with MPLS VA security, access, and badging requirements. Employees must be able to pass a background check so they can obtain a Tier Level 1 PIV Security access badge. Contractor s employees will submit fingerprints and all required documents within five (5) business days of Notice to Proceed (NTP) or award date of contract. Contractors are required to check in with the COR/POC or designated representative upon arrival at facility to obtain instructions, work orders, and project location information. Daily performance log sheets are required to be submitted to the COR/POC or designated representative weekly. Parking/Storage: Parking is available in parking lots 11, 43A, and 43B for vehicles (automobiles and Pickup trucks). Larger vehicles, equipment, job site trailers, or storage containers must utilize parking lot 43A only. The Contractor s contact information (name and phone number) is required to be displayed on all vehicles, equipment, trailers, or containers that are stored overnight at the MPLS VAMC. Dropping off tools and materials at the loading dock is permitted, but no parking is allowed in the loading dock area. Unattended vehicles in unauthorized parking areas will be ticketed by MPLS VAMC Police. Contractors Proposal: Contractor s proposal must include the following: Provide Company s Capability Statement. Company must have at least five (5) years of experience moving office furniture. Identify Key personnel and their roles. Provide employees resumes that show five (5) or more years of experience performing moving services. Provide references from three or more establishments where Contractor s services were performed at this magnitude. Provide quality control/quality assurance plan. Provide proof of insurance that will cover property damage. Complete Rate table below for services based on the estimated hours listed and submit with proposal.
