Page 11 of 11 Dayton Tool Sharpening Questions and Answers: The Statement of work does not require device repair, but referencing the inventory and the Evaluation Factors criteria #5, is there a need for device repair for cameras, rigid scopes and power dental equipment or is this strictly an onsite/offsite maintenance and repair for stainless surgical instrumentation? This is for surgical/dental stainless steel and laparoscopic instrumentation. The items in question are covered under an existing contract and would not be included in the scope of this work. Dayton VAMC already has a contract # 36C25023P1540 for Power Equipment, we are assuming the power equipment inventory in this solicitation is in addition to the items covered under that contract? Correct, in addition. If device repair is necessary are some of the scopes duplicated in the Inventory Scope Listing and then again in the Surgical Set/Instrument listing? There won t be any need for a scope repair. We will be updating the inventory list for the new solicitation that will be posted. Can you provide a listing of case carts or at least the number of carts requiring maintenance and repair? We have 48 new Pedigo Revolution case carts, 5 AMSCO sterilizer carts. This is an infrequently repaired item. Typically, 1-2 repairs per year. Can you clarify the PoP. There is a discrepancy, page 5 Performance Work Statement states base + 4 options, and page 40 above the Evaluation Factors, states base + 2 years? Base and 2 years is the correct POP. Please clarify the need expressed in Evaluation Factor #3. This item is being amended in the new solicitation posting. We will be changing to one business day to initial call response. In regard to the question this deals with the 24 hours response time under emergency repairs and we want to know what mechanisms are in place to handle this type of situation. This would be avoiding a week or so delay for the next planned visit. Section 1: General Information 1.3 states Dayton VA Medical Center and Off-site location. Is there 2 locations that require services or are you referring to the Vendors repair depot as the off-site location? On-site at Dayton and off-site is referring to vendor facility. Can you provide the model numbers of your device inventory? We will address this by removing these items in the new solicitation. This will only pertain to the stainless steel and laparoscopic items. Will there be any instrument marking requirements, laser etc.? No, none needed, we have our own laser etcher. Is there an average tray count that needs to be serviced at each visit? 6-8 sets along with assorted single one-off instruments needing repair. PWS and price cost schedule state instrument repair and sharpening, offsite instrument repair, and autoclave cleaning, but does not mention scopes, cameras, autoclave repair, and dental power equipment.  Which can be interpreted as having the capability, but quoting the additional services at T&M. Should our bid only include the price for onsite instrument repair, offsite instrument repair, and autoclave cleaning annual cost?  VA does not have the option for T&M on our contracts, getting an answer on inclusion. Would need quoting as a FFP. Yes, confirmed those three items are what would be bided for this contract. (Onsite instrument repair, offsite instrument repair, and autoclave cleaning annual cost) Or, also include the annual cost for all scopes, cameras, dental power equipment, and autoclave repairs?  The cost would be dramatically different. These items are being removed from the contract and the new solicitation will have the updated list of surgical and laparoscopic tools. Evaluation Factors, pg 40 states autoclave cleaning annually, but price/cost schedule says twice annually.  Which is it? We are revising with the updated PWS to bi-annually (2X) cleanings per year are needed.  Case Cart repairs are not in the PWS. Is the desire to have the service done monthly and billed T&M?  VA does not have the option for T&M on our contracts. We have 48 new Pedigo Revolution case carts, 5 AMSCO sterilizer carts, might need 1-2 repairs per year, very rarely need repairs.  Evaluation Factors, pg 40 states autoclave cleaning and repairs, but the price/cost schedule and Section 5 pg 9 only states cleaning.  Which is it? Cleaning only. Section 1, pg 5 states this contract is for base period with four (4) options years, but the price/cost schedule is for base period with 2 option years. What is the actual length of the contract? Base and 2 options is the correct timeframe, will amend and revise on new solicitation.
