SPECIAL NOTICE FOR QUALIFICATIONS (SF-330) NAICS Code: 541330 Engineering Services (Small Business size standard $25.5 million dollars) Set-Aside: This project will be a 100% Small-Disadvantaged Veteran-Owned Small Business (SDVOSB) set aside. This is not a Request for Proposal (RFP): This is a request for SF 330's Architect/Engineer Qualifications packages only. Any requests for a solicitation will not receive a response. No material will be issued, and no solicitation package or bidder/plan holder list will be issued. INTRODUCTION: The purpose of this Special Notice for Qualification is to provide instructional guidance to assist firms with completing their Architect-Engineer Qualifications SF 330 and any supporting documentation. This is a request for SF 330 Architect/Engineer (A/E) Qualifications packages only. All submissions will be evaluated in accordance with the selection criteria described in this notice. The Government will neither pay, nor reimburse, any costs associated with responding to this SF 330 Notice. The Government is under no obligation to award a contract as a result of this announcement. Respondents are put on notice that firm(s) selected by the Selection Authority will be required to submit a Fixed Price Proposal on the VA Form 6298 to include labor, overhead, profit, and other costs for this project for negotiation by the Contracting Officer prior to contract award. The proposal accepted by the VA will be incorporated into the resultant contract. GENERAL CONTRACT INFORMATION: The Department of Veterans Affairs, VA Providence Healthcare System (VAPHS), 830 Chalkstone Avenue, Providence, RI 02908 has a requirement for a Firm Fixed Price contract to provide professional Architect/Engineer (A/E) Design Services necessary to Renovate 6A Medical Surgical Inpatient Nursing Unit, to include but not limited to investigative services, design site surveys, acquiring field data, and verifying as-built conditions to ensure accurate development of design and bidding documents, space programming, schematic design, design development, construction documents, specifications, cost estimates, construction period services and site visits. The Medical Surgical Inpatient Nursing unit in Wing 6A has become functionally obsolete. The unit does not meet the current VA Providence HealthCare System design guides and manuals. This project will reconfigure the wing to provide a modern room arrangement to address privacy, dignity, and security deficiencies to bring space to comply with current VA design criteria. All project work will be coordinated through VAPHS staff to minimize disruptions to daily operations and activities. The A-E shall provide design & construction documents and construction period services for Renovate 6A Medical Surgical Inpatient Nursing Unit of the Providence VAMC. Project Number 650-24-104. MAGNITUDE OF CONSTRUCTION: The Magnitude of Construction is between $5,000,000 and $10,000,000. SET-ASIDE INFORMATION: This acquisition is a Total Set-Aside for Service-Disabled Veteran Owned Small Business (SDVOSB) in accordance with VAAR 852.219-73 with NAICS Code 541330. Any subsequent award will require the firm to be registered in System for Award Management (SAM) website accessible at https://sam.gov/content/home and certified in Veteran Small Business Certification (sba.gov). The above Veteran Small Business Certification site will be used by the VA to determine each Firms SDVOSB status at time of submission of qualifications. To be eligible for evaluation and selection, each Firm must be listed as certified or otherwise registered (JV) at the time they submit their SF 330 package to the VA. LIMITATIONS ON SUBCONTRACTING: Prospective firms are reminded they must certify that, in accordance with VAAR 852.219-75, VA Notice of Limitations on Subcontracting-Certificate of Compliance for Services and Construction, at least 50 percent of the billable work under the contract will be performed by the Firm or similarly situated concerns. SF 330 packages shall address how each firm anticipates meeting these criteria. This requirement is in accordance with the Selection of Architects and Engineers as implemented in Subpart 36.6 of the Federal Acquisition Regulation and Subpart M836.602-1 of the VA Acquisition Manual. All submissions will be evaluated in accordance with the selection criteria. A ""short list"" of the three (3) or (4) most highly rated Firms after initial slating board review will be chosen for interviews, and the Government intends to negotiate fair and reasonable design fee with the top-rated firm in accordance with FAR Subpart 36.6. SELECTION CRITERIA: Each respondent s SF 330 submittal package will be evaluated in accordance with FAR 36.6 procedures. The selection criteria, in descending order of importance will be as follows: Professional Qualifications Specialized Experience and Technical Competence Past Performance Capacity Geographic Location Construction Period Services Experience SELECTION CRITERIA DESCRIPTIONS: A description of the six (6) Selection Criteria that each Firm shall respond to within their SF 330 submittal are provided below: PROFESSIONAL QUALIFICATIONS: The A/E firm will be evaluated according to their ability to demonstrate the education, training, registration, overall and relevant experience, and longevity with the firm of key management and technical personnel. Firms are expected to demonstrate experience to perform a project such as this within a VA Providence Healthcare System or hospital environment. Each A/E firm must submit Professional Qualifications necessary for the satisfactory performance required A-E services in the SF 330 submission by demonstrating the requirements in this section and SOW: Identify the proposed team that will address the A-E firm along with any key and/or frequent Subcontractor that will perform technical tasks for the at-issue project. Provide an organizational chart that displays the proposed team of the A-E Firm, any other Firms, and any key or frequent Subcontractors that will perform technical tasks for the at-issue project.  Provide brief resumes of each proposed team members, who will specially serve as Supervisor(s), Project Manager(s), and Designer(s) of Record and/or frequent subcontractor (s) from the A/E firm that describes the professional qualifications associated with this project. All proposed team members/personnel and supervisors shall possess the minimum education, training, experience, registrations, and certifications. Each brief resume shall feature a minimum three (3) specific completed projects with three (3) to five (5) years of experience that best illustrate the individual team members experience relevant to this contract scope and each team member/personnel performed a signification role that will be comparable to the at-issue Renovate 6A Medical Surgical Inpatient Nursing Unit Project. In addition, provide resumes of other relevant team members or frequent subcontractor who will perform technical tasks under this contract including their record of working together. The Designers of Record shall perform and direct the design within their respective discipline and shall sign and seal the drawings. All Designers of Record must be professionally registered in the discipline. Additionally, the firm shall address the criteria for meeting the Limitations on Subcontracting. Provide proof of jurisdictional licensure with either a license or license number for each. The Designer of Record from the A/E Firm that will represent the project and/or sign/stamp drawings. Each Designer of Record must be licensed, certified, or registered in a State of the United States, the District of Columbia, or a United States territory for a minimum of three (3) years. The proof of jurisdictional licensure with a license or license number may be included within the brief resumes as discussed in the preceding paragraph of this section.   Describe the Professional Qualifications for satisfactory performance of the required A-E services including, but not limited to, the following areas of discipline:     Architecture Interior Designer Structural Engineering Mechanical Engineering Electrical Engineering Plumbing Engineering Fire Protection Engineering Information technology/Telecommunications systems Specialty Work Cost Estimating Hazardous Materials Abatement Any other design services and specialty design services not specifically listed above that is needed to provide a complete and thorough design.   Specialty Work, for purposes of this Professional Qualifications section, is defined as:    Firm with extensive expertise in Design of Medical Surgical Inpatient Nursing units.   Minimum of three (3) design of Medical Surgical Inpatient units within the last five (5) years.      The A-E Firm shall exhibit at least five (5) years of experience in each of the twelve (12) aforementioned areas of discipline.  NOTE: Proposed team members/personnel shall also be team members/personnel of the award team. A substitution of any team member/personnel must receive prior written consent of the Contracting Officer.  SPECIALIZED EXPERIENCE AND TECHNICAL COMPETENCE: Each A-E Firm must submit Specialized Experience and Technical Competence in the type of A-E work required in the SF-330 submission by demonstrating the requirements in Paragraphs (a) through (c) in this section. The A-E Firm shall also review the Specialized Experience and Technical Competence factors considered in Paragraph (d) of this section: Provide a detailed narrative that contains no more than three (3) (maximum) relevant A-E Design Services projects performed and completed within the last five (5) years. The three (3) A-E Design Services projects selected by the A-E Firm should best illustrate the relevant team experience similar in the scope and the nature of this Renovate 6A Medical Surgical Inpatient Nursing Unit project. Said narrative that comprises of the three (3) most relevant A-E Design Services projects should indicate Government experience or private sector experience in Medical Surgical Inpatient Nursing units of similar size, expertise in new design guidelines and requirements. The three (3) projects shall be of similar in size, scope, and complexity. These three (3) most relevant projects selected by the A-E Firm for this detailed narrative must entail the A-E Firm s ability to successfully execute the requirements identified in the attached Statement of Work.   Include within this Specialized Experience and Technical Competence section similar A-E Design Services projects to the Renovate 6A Medical Surgical Inpatient Nursing Unit Project, which have been previously performed and completed by the A-E Firm. Include within this Specialized Experience and Technical Competence section any knowledge of similar projects previously performed and completed by the A-E Firm in a hospital setting or Veteran Affairs setting that required the following components: Architecture Structural Engineering Mechanical Engineering Electrical Engineering Plumbing Engineering Fire Protection Engineering Information technology/Telecommunications systems Specialty Work Cost Estimating Hazardous Materials Abatement Any other design services and specialty design services not specifically listed above that is needed to provide a complete and thorough design. d. The Selection Board may also consider the A-E Firm s strategic business plan under the following categorizations: Management Approach Quality Control Procedures Planning Information Standards Coordination of Disciplines and Subcontractors Prior Experience of the A-E Firm Prior Experience of Subcontractors selected by the A-E Firm; and Collaboration with Subcontractors selected by the A-E Firm PAST PERFORMANCE: Firms will be evaluated for the past performance on recent and relevant contracts with Government agencies (Emphasis on VA work) and private industry in terms of cost control, professional performance, general management, cooperativeness, quality of work, compliance with performance schedule and a record of significant claims against the firm due to improper or incomplete engineering services (references required). Each A-E Firm shall submit three (3) Past Performance on contracts with Government agencies and private industry in the SF 330 submission. The A-E Firm shall provide and demonstrate the Past Performance requirements set forth in Paragraphs (b) and (c) of this section. The A-E Firm shall also review the Past Performance descriptions and definitions in Paragraph (a) of this section as well as the factors considered in Paragraphs (d) and (e) of this section.   Past Performance, under FAR 36.602, is described as contracts with Government agencies and private industry for recent and relevant projects in terms of cost control, quality of work, and compliance with performance schedules.  Recency, for purposes of this Special Notice, is defined as any contract performance occurring within five (5) years of the date of this Special Notice; Ongoing projects must have begun no less than one (1) year prior to the issuance of this Notice. Relevancy, for purposes of this Special Notice, is defined as past performance of work on projects that are similar in scope and complexity to the type of project anticipated under the resultant contract. Respondents with no previous past performance shall so state when addressing the selection criteria. Documentation supplied should detail firms ability to work with government entities and design standards or similar relevant experience with other entities. Provide Recent and Relevant Contract Performance Assessment Reporting System (CPARS) data or PPQs (if no CPARS date is available) must be submitted with Firm s SF 330 package. CPARS data and PPQs do not count toward the page limitation Government Past Performance Information Systems may be accessed. Provide a copy of all performance evaluations (A/E) issued for government contracts (as applicable). For non-Federal contracts, provide facility owner documentation of the firms performance issued on that contract. If known documentation exists, so state. Provide the following information for each Recent and Relevant Past Performance contract: Accessible owner points of contact: name, title, address, telephone number, e-mail, and provide copies of awards and letters of appreciation/recommendation as desire. Contract Number and/or Task Order Number  Obligation Number/Purchase Order (PO) Number  Project Title Prime Firm Name Start Date Completion Date; and  Construction Dollar Amount.   The Government also reserves the right to evaluate Past Performance on previously awarded Federal Task Orders and any other information available. Additionally, the Government may review the Firm s previous ability to meet the 50% SDVOSB performance requirement on other Task Orders.  Performance criterion may also include (i) additional performance related from the A-E Firm, (ii) customer inquiries, (iii) Government databases, (iv) publicly available sources, and (v) additional projects in CPARS other than in Paragraph (b) of this section.  It is recommended that each Past Performance reference provided to the Government may be notified by the A-E Firm that a Contracting Officer will contact each Past Performance reference.  Past Performance Questionnaires (Attachment 1 to this Notice): Respondents shall complete Section A (General Information) of the Past Performance Questionnaire and provide it to NCO 1 representative as part of the SF 330 submittal package. Respondents shall submit the Past Performance Questionnaire Sections B through D and the Cover Letter to each Contracting Officer (or customer contact equivalent) and Project Manager (or customer contact equivalent) identified in its SF 330 Section F, Example Projects Which Best Illustrate Proposed Team s Qualifications for this Contract . Sections B through D of the questionnaire shall be completed ONLY by the contract customer. The contract customer shall complete sections B through D of the Past Performance Questionnaire and shall submit it directly to NCO 1 by the SF 330 submittal package due date identified in the Notice. Past Performance Questionnaire Sections B through D will not be accepted from Respondents. Questionnaire sections B through D must come directly from the Respondent s contract customer. The Respondent bears the burden of providing relevant and timely references with accurate and current telephone numbers and addresses. The Respondent is responsible for ensuring that the questionnaires issued by them are completed by the contract customer and submitted directly by the contract customer to NCO 1 no later than the SF 330 submittal package due date designated in this Notice. Contract customer shall E-mail Past Performance Questionnaire Sections B through D. Respondents are notified that NCO 1 may use any additional sources of information available to it regarding the Respondent s relevant experience and past performance and consider the information thus obtained as part of this evaluation. CAPACITY: Each A-E Firm must submit the Capacity to accomplish the work in the required and allotted amount of time in the SF 330 submission by demonstrating the requirement in Paragraph (a) of this section. The A-E Firm shall also review the Capacity factors considered in Paragraph (b) of this section. This factor includes determining customer satisfaction, project cost, and meeting project schedule by: Provide the available capacity in the five (5) required areas of discipline for this project, as previously denoted in Paragraph (c) of the Professional Qualifications section, by furnishing current project workload that includes all projects awarded during the previous twelve (12) months and the full potential value of said projects.     The Selection Board will consider the A-E Firm s plan and ability to fulfill the schedule and timelines of the overall project. The Selection Board will also consider the available capacity of the A-E Firm to provide adequate personnel that properly execute the five (5) key disciplines of the at-issue project.   GEOGRAPHIC LOCATION: Each A-E Firm must submit information relating to the general geographical location area of the project and locality knowledge of the project in the SF 330 submission by demonstrating the requirements in Paragraphs (a) through (c) in this section.   Provide the name and address of the A-E Firm s nearest branch or satellite office in geographic proximity to the Providence VAMC located in Providence, Rhode Island.     Provide familiarity with the Providence VAMC. Familiarity with the Providence VAMC may constitute the following components:   Travel to and from the location of the Providence VAMC.   Travel through the surrounding areas of the New England region.   Climate of the New England region.  Major roadways surrounding the metropolitan area of Providence, Rhode Island; and  Other sufficient transportation methods through Providence, Rhode Island.  The locality knowledge element of this project is essential, which may be affected by the climate, roadway conditions, and unforeseen circumstances in the New England region.     Provide any A-E Design Services projects previously completed by the A-E Firm for the Government within the past two (2) years at the Providence VAMC or in the New England region of NCO 1 (Connecticut, Maine, Massachusetts, New Hampshire, Rhode Island, and Vermont). For each A-E Design Services project previously completed within the past two (2) years at the Providence VAMC or in the New England region of NCO 1, provide the following project details:   Contract Number and/or Task Order Number.  Obligation Number/Purchase Order (PO) Number.   Project Title.  Start Date; and  Completion Date.  CONSTRUCTION PERIOD SERVICES EXPERIENCE:  Each A-E Firm must submit information relating to Construction Period Services Experience in the SF 330 submission by demonstrating the requirement set forth below in this section.     Describe Construction Period Services Experience for projects similar in scope and complexity, which may include, but is not limited to, the following aspects:   Professional field inspections during the construction period.  Review of construction submittals and/or requests for information, shop drawings.  Support in answering requests for information during the construction period; and    Support of construction contract changes to include drafting statements of work and cost estimates.  QUESTIONS AND COMMENTS: Questions and comments regarding this notice must be submitted by email only to the Contract Specialist Kimberly Gomez at kimberly.gomez@va.gov. Oral questions will not be answered. SF 330 SUBMITTAL PACKAGE PAGE INSTRUCTIONS: The entire SF 330 submittal packages, including Past Performance Questionnaires, shall be submitted to the VA no later than 1:00 PM EST on November 28, 2023. Respondents are responsible for ensuring SF 330 submittal packages, any revisions, and modifications are submitted by this time. All interested and qualified A/E firms are required to submit ONE (1) electronic copy via email to Kimberly.gomez@va.gov. If file size exceeds 7MB then firms are required to send multiple emails with a sequence stating email 1 of __ . Current SF 330 forms are available on-line at: https://www.gsa.gov/forms#SF Submission of information incorporated by reference is not allowed. Prospective firms shall address all selection criteria factors within their submitted SF 330 package. Please be specific, clear, and concise with all response information related to Selection Criteria described under Sections F and G (i.e., please do not merely state that information is incorporated by reference ). The entire SF 330 package is limited to fifty (50) pages. With the exception of the below items, pages that exceed the Fifty (50) page limit will not be considered or evaluated by the Selection Board. The following items are excluded from the fifty (50) page limitation: Cover letter Table of contents Past Performance Questionnaire submittals Do not include photographs with SF 330 submittal; however, drawings may be submitted if needed to support any portion of Firms SF 330 submittal. Pages submitted in excess of the limitations specified in this Notice will not be evaluated by NCO 1. Telephone or Fax inquiries will not be accepted. SF 330 PACKAGE CONTENTS: The SF 330 submittal packages shall consist of a completed SF 330 parts I and II, specific information addressing each of the SIX (6) selection criteria described in this Notice; the additional information required by this Notice and past performance questionnaire section A. Respondents are required to meet all requirements in addition to those identified as selection criteria. SF 330 submittal packages not consistent with the requirements and selection criteria of this Notice may be determined unacceptable and removed from consideration. Any SF 330 submittal package, modification, or revision, that is received at the designated NCO 1office after the exact time specified for receipt is late and will not be considered unless it is received before award is made, the contracting officer determines that accepting the late submittal would not unduly delay the acquisition; and There is acceptable evidence to establish that it was received at NCO 1 installation designated for receipt of submittal packages and was under NCO 1 s control prior to the time set for receipt of submittal packages; or it was the only submittal package received. SF 330 PACKAGE PREPARATION: Qualified A/E firms are required to submit ONE (1) electronic copy via email to kimberly.gomez@va.gov SF 330 forms are available on-line at: http://www.gsa.gov/portal/forms/type/SF Electronic copy shall be in searchable PDF format of the SF 330 parts I and II, questionnaire, CPARS and additional information requested by this Notice. Cover letter and at a minimum must include the following: Completed SF 330 package and resumes. Firm s Legal Name, and DBA (if applicable), and full physical address, can also include mailing address. Firm s SAM UEI Response Date (date Firm s submits its SF 330 package to the VA). Number and Title of this Notice. A cover sheet should be included, clearly marked with the Respondent's name, complete address with 9-digit zip code, phone number, e-mail address, SAM UEI, date of response, title, and Notice identification. Information not provided elsewhere in the SF 330 submittal package shall not be incorporated by reference. A table of contents shall be provided for ready reference to sections and figures. SF 330 submittal packages shall be consistent with the following structure and formatted accordingly: A completed VAAR 852.219-75, VA Notice of Limitations on Subcontracting-Certificate of Compliance for Services and Construction.
