Description

This amendment reflects clarification to questions pertaining to RFQ 36C26224Q0159 Home Oxygen Therapy Equipment for the VA Phoenix and VA Prescott Healthcare Systems. Please refer to the questions and answers below. Question 1: Pertaining the Delivery Schedule. Will this be an all or nothing bid, that would have to include the ""Planning to Order"" items (ResMed and Respironics)? Answer 1: You must be able to deliver all of the total quantities for each of the items listed on the delivery schedule for both warehouses listed on the delivery schedule. The Planning to Order items (ResMed and Respironics) have been removed from the delivery schedule. Question 2: Can you please provide clarification on the following documents. Attachment A has 11 CLINs listed and Attachment C worksheet has 11 CLINS. The Attachment B Delivery schedule with quantities has 10 CLINS listed, (I believe the IGO2 is missing) but then a few extra items on the bottom not related to the contract. If you could please update attachment B for the delivery schedule and quantities? Answer 2: The Previous Amendment 0001, updated the Delivery Schedule with the missing Drive iGO2 Portable Oxygen Concentrator Kit w/extra battery pack. The Delivery Schedule, as well as the Price Schedule have been updated to reflect the changes mentioned above. Question 3: What is the quantity and delivery schedule for the Drive iGO2 Portable Oxygen Concentrator Kit w. extra battery pack Manufacturer DeVilbiss Healthcare Part # 125D-XB? Answer 3: The total quantity for this item is 1,938, this was updated in the Price Schedule Attachment C and in the Delivery Schedule Attachment B in the previous Amendment 0001. Roughly 2/3 of the order will to be delivered to Mid-Cities in a 3-month time span from December through February. The remaining 1/3 of the total will be delivered to Calox, shipping early December through January. Question 4: After awarded how soon do you want the product for the first delivery? 15 ARO, 30 ARO 45 ARO? Answer 4: 15 ARO or sooner if possible.
