This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 12.6 in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. This combined synopsis/solicitation is issued as a Request for Quote (RFQ) 36C24824Q0202. Submit written quotes in accordance with Addendum to FAR 52.212-1, Instruction to Offerors. Oral quotes will not be accepted. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC-2021-05 / 03-10-2021. North American Industrial Classification Standard (NAICS) 561621, Security Systems Services (except Locksmiths), with a Size Standard of $22.0M. Miami VA Healthcare Network intends to award a single firm fixed price contract for Video Surveillance Cameras Security System Service. This procurement is a 100% Total Service-Disabled Veteran Owned Small Business (SDVOSB) Set-Aside and all eligible quoters are encouraged to submit quotes. To be eligible for award quoter must be Verified in the Vendor Information Pages (VIP) (https://www.vip.vetbiz.gov) and be registered with the System for Award Management (SAM) at the time of the close of the RFQ and at the time of award. Introduction. 1.1. The Government requires a video surveillance camera service that includes preventive service, repair, replacement, and a condition-based maintenance contract, to ensure productivity, efficiency, and uninterrupted business operations that protects against data loss, unscheduled outages, and downtimes. This proactive service, repair, replacement contract and prevention requirement have the benefits of developing audit history, minimizing unscheduled break/fit work, assures regular maintenance scheduling, and identifies/mitigates known risks to the Miami VA Healthcare System, Police Service. 1.1.1. The term of this agreement shall be from the effective date and will be automatically renewed for one-year thereafter; the total duration not to exceed 60 months. 1.2. The objective is to provide service, repair, or replacement to malfunctioning equipment and to complete a quarterly preventative maintenance inspection as outlined in the Manufacture s Owner/Installation Manual(s) for all medical center Security Surveillance television (SSTV) cameras and equipment, and to provide emergency repair support for equipment, to always maintain a complete operating system. 1.2.1. The annual agreement cost for each succeeding year will have a mutually agreed annual increase. 1.3. The Contractor must be Safety Act Certified. 1.3.1 The Operating System software, Cameras, NVRs and Monitors must be Pelco based. 1.4. Preventive maintenance service, repair, and replacement: Description /Scope/Objective 2.1. The Contractor shall be responsible for conducting quarterly maintenance inspections of all SSTV equipment (See Appendix A) 2.1.1. The Contractor must provide all labor, equipment, tools, material, supervision and other items and services necessary to perform the work as defined in this Performance Work Statement. 2.2. An initial servicing of all SSTV equipment will be performed within 60 days of contract award. Upon completion, all equipment shall be inspected, calibrated, land tested once (1) per quarter. Cleaning of all cameras, lenses, and housing with a lint-free cloth (if a lift is required to access exterior cameras the equipment will be provided by the contractor). Sealing of exterior cameras where necessary with weatherproof sealant. Servicing of all recording devices, DVRs, Workstations, or servers, and ensuring programming and software updates if required. Including removing dust accumulation and ensuring that devices are programmed and recording properly. Inspection of Console to ensure proper video feed for cameras. Upon completion of Preventative Maintenance, a complete service report will be provided, as outlined under the reporting section of this performance work statement; the preventive maintenance service report shall include: (a) issues detected through condition monitoring, and (b) routine inspections uncovering potential faults leading to equipment break downs for the purpose of minimizing corrective maintenance service. 2.2.6a. The Government anticipates Corrective Maintenance servicing for equipment covered by this agreement, except as subject to conditions stated under Exclusions , and will provide a purchase order to Contractor, prior to work commencing if the equipment is not covered. 2.2.6b. The Contractor will repair, replace parts and/or assemblies, or replace equipment if it is unrepairable with replacement model of same equipment, whether existing or new as covered by this agreement, to return equipment to its operating condition. 2.2.7. The Contractor will provide all necessary equipment for the completion of scheduled maintenance. 2.2.7a. Equipment will not be out of service more than five (5) business days for completion of Preventative or Corrective Maintenance. 2.2.8 Contractor must pre-notify the COR or designee if work is to be conducted during non-normal duty hours must be approved in writing from the Contracting Officer s Representative no less than 72 hours in advance. (i.e., evenings, nights, weekends, holidays, etc.). 2.3. The Contractor shall be required to provide emergency service 24-hours a day, 7 days a week. This requires unlimited emergency callback service for emergent repairs, problems or systems failure. 2.3.1. The contractor shall be required to report within four (4) hours of notification to the appropriate Medical Center. Notification can be accepted by the Contractor from the following staff members for emergent repairs only: COR, Dispatch Supervisor, Physical Security Specialist, Deputy Chief of Police or Chief of Police, in that order. Contractor will report to the appropriate location, upon arrival contractor will check in with the dispatch section via phone at (305) 575-3400. 2.3.2. A Contract Line Item shall be incorporated in each year equivalent to (20%) twenty percent of the contract cost for emergent repair and additions of the SSTV. This line item will be utilized throughout the year to invoice for services rendered on emergent repairs or additional equipment required for the SSTV. Contractor cannot exceed this amount unless authorized in writing by the Contracting Officer. The COR shall monitor the funding under this line item and notify CO if the line item is encroaching the amount limit. 2.3.3. All emergent repairs and/or additions estimates shall be submitted in writing and shall be approved in writing by the COR or their designated appointee prior to commencement of the work. Any additional work performed by the contractor without the prior written approval of the COR will be performed by the contractor at no additional cost to the government. 2.3.4. Emergency Service: Emergency repairs beyond the scheduled maintenance and for locations that are considered emergency areas as follows: Miami VA Medical Center Emergency Room, Mental Health Ward and 5th Floor. The contractor shall respond to a call and be at the job site within four (4) hours, twenty-four hours a day, seven days a week, (365) three hundred sixty-five days a year at no additional cost to the Government. 2.3.5 All Emergency Repairs will be completed by the contractor within 5 business days. Exceptions may only be approved by the COR. 3.0. Applicable Documents. 3.1. Performance Requirements. 4.1. The Contractor shall provide: Video Surveillance Cameras services and maintenance for all areas identified herein. The contractor shall identify entities attempting to subvert in-place security controls, identify threats and assure protection measures essential to the safety and security of Government in accordance with VA policies, procedures, and any other Governing doctrine thereof legal, industry, Federal/State/Local or otherwise. Shall be licensed/qualified to perform maintenance on the PELCO System; DS Control Point Software. 4.1.1. At the convenience of the Government the Contractor shall coordinate any and all services necessary to maintain the system(s). Services consist of but are not limited to the following items: 4.1.1.1. Inventory and recording of system components. 4.1.1.2. Installation, preventive service, repair, replacement, monitoring, inspection, and streaming video/SSTV. 4.1.1.3. Testing power supply for monitoring systems. 4.1.1.4. Testing cables, failure determinations and eliminations. 4.1.1.5. Testing all detectors and operation tests of all signal units of the system. 4.1.1.6. Preparation of maintenance and functional test records for the monitoring system. Failures must be listed in detail. 4.1.1.7. Any possible repair, replacement, or preventive services are included in this contract. This is a turnkey contract resulting in the contractor bearing full responsibility of the system for possible repairs. 4.1.2. On-call Repairs: On-call repairs are emergency repairs beyond the scheduled maintenance. 4.1.2.2. Upon any and all malfunctions the contractor shall perform services immediately on the same day. At no additional costs an immediate service request can be requested after 1630 hours, weekends, or any Federal holidays. 4.2. Contractor s exclusionary clause(s) shall align the Federal Acquisition Regulation (FAR) guidelines. 5.0. Performance Location and Hours. 5.1. Government normal duty hours are from 0730-1630, Monday through Friday, excluding federal holidays. Contractor access will be provided during this time. 5.2.1 Normal working hours shall be used to complete this contract, unless otherwise indicated. Deliverables. 6.1. The contractor shall conduct instructional course(s) to meet the objectives identified herein. In addition, the contractor shall provide handouts and training materials in sufficient quantity for each participant. These are off-the-shelf courses, and the contractor shall provide lesson plan, course materials, and media (DVD s, CD s, videos, etc. if required) for each course. 6.2. The contractor shall perform all management tasks that are required to perform the tasks identified below in accordance with the established schedule and to report the status/progress of these tasks to the local VA point-of-contact and the VA COR. 6.3. The contractor shall develop and maintain a project plan that identifies tasks to be performed, identify the resources, facilities, tools, and materials needed to effectively accomplish tasks. The plan shall explicitly include task dependencies so that the impact of a delay in completing any task can be readily identified. 6.4. The contractor shall provide, schedule, supervise, and manage all personnel required to perform the tasks required in this performance work statement. 6.5. The contractor shall ensure that all personnel are properly trained and licensed. 6.6. The contractor shall provide all service reports. 6.7. The contractor shall provide a comprehensive report to the VA COR on a quarterly basis, containing the following information date service was completed; what service was completed for each piece of equipment identified in this Performance Work Statement (see Appendix A), and required software updates and date(s) of installation; all documentation to remain the property of the Government. 7.0. Key Personnel, Replacement and Substitution. 7.1. Requests for approval of substitutions shall be in writing and shall provide a detailed explanation of the circumstances necessitating the proposed substitutions. The request shall contain a complete resume for the proposed substitute, and any other information requested or required by the Contracting Officer to approve or disapprove the request. Proposed substitutes shall have qualifications that are equal to or higher than the personnel being augmented. The Contracting Officer or his/her authorized representative shall evaluate such requests and promptly notify the Contractor in writing whether the proposed substitution is acceptable. 7.2. If the Contracting Officer determines that, (1) suitable and timely replacement of personnel who have been reassigned, terminated or have otherwise become unavailable for the contract work is not reasonably forthcoming, or (2) the resultant substitution would be so substantial as to impair the successful completion of the contract or the delivery order in accordance with the proposal accepted by the Government at time of contract award, the Contracting Officer may, (1) terminate the contract for default or for the convenience of the Government, as appropriate, or (2) at his discretion, if he finds the Contractor at fault for the condition, equitably adjust the contract price downward to compensate the Government for any resultant delay, loss or damage. 7.3. The Contractor shall replace any absent individual for any period greater than one workday (may be changed) so as to leave a full complement of trained qualified personnel at all times with no disruption of services. 8.0. Contractor Personnel and Management. 8.1. Contractor Personnel. The Contractor shall be responsible for employing technically qualified personnel to perform the work specified in this PWS. The Contractor shall maintain the personnel, organization, and administrative control necessary to ensure that the work delivered meets the contract specifications and requirements, as follows: 8.1.1 Provide, schedule, supervise, and manage all personnel required to perform the tasks required by this performance work statement. 8.1.2 Ensure that all personnel are properly trained and licensed for work on the PELCO Surveillance System; all contractor personnel shall comply with facility requirements for sign-in and badge display. 8.1.3 Coordinate all requirements for escorts with the local COR at least one week in advance of required access. 8.1.4 Be responsible for any delays or other impact due to misconduct on the part of its employees or subcontractors, and promptly remove and replace any of its personnel who do not comply with VA requirements. 8.2. Contractor Badges. Notwithstanding the requirement to wear a Government furnished badge, Contractor personnel shall also always wear a laminated contractor identification badge when performing services under this Contract, including when on Government directed travel. The badge will contain a personal picture, name of employee, and Contractor's name. Badges shall be worn on the outer garment attached to the outer shirt or jacket pocket by a button or clip or worn around the neck secured by an appropriate identification card lanyard. 8.3. All Contractor personnel attending meetings, answering Government telephones, and working in other situations where their contractor status is not obvious to third parties are required to identify themselves as such to avoid creating an impression in the minds of members of the public or Congress that they are Government officials. They must also ensure that all documents or reports produced by Contractor personnel are suitably marked as Contractor products or that Contractor participation is appropriately disclosed. 8.4. Safety/Security Requirements. The Contractor shall comply with all applicable federal, state and local laws and ensure performance is secure while protecting material, equipment, and supplies from damage and loss. Government security personnel shall have the express right to inspect for security violations at any time during the term of the contract. 8.5. General Requirements 8.5.1 All employees of the vendor and subcontractors shall comply with the VA security management program and obtain permission of the VA police, be identified by the contract and employer, and restricted from unauthorized access. This will be coordinated with the VA Police by the contracting officer s representative (COR). 8.5.2 The vendor s employees shall not enter the work site without appropriate badge. They may also be subject to inspection of their personal effects when entering or leaving the project site. Badges shall be provided by the Government to the vendor for distribution to on-site employees of the vendor and all Subcontractors. The vendor shall maintain a log of Badge ID numbers and names of employees the badges were issued to. Updated badge log shall be given to the COR weekly or less frequently if directed by COR. Lost badges shall be reported immediately to the COR and the General Contractor will be responsible for a $10.00 fee for all lost or unreturned badges. 8.5.3 No photography of VA premises is allowed without written permission of the Contracting Officer s Representative. 8.5.4 Site and Building Access: Maintain free and unobstructed access to facility emergency services and for fire, police, and other emergency response forces in accordance with NFPA 241. 8.5.5 Any electrical work required for the maintenance, repair or installation of equipment must meet the International Building Code (IBC), National Electrical Code (NEC), Underwriters Laboratories, Inc. (UL) and National Fire Protection Association (NFPA) as minimum installation requirement standards. 8.5.6 Materials and equipment furnished shall be of current production by manufacturers regularly engaged in the manufacture of such items, for which replacement parts shall be available. 8.5.7 All electrical work must comply with the requirements of NFPA 70 (NEC), NFPA 70B, NFPA 70E, OSHA Part 1910 subpart J, OSHA Part 1910 subpart S and OSHA Part 1910 subpart K in addition to other references required by contract. 8.5.8 New work shall be installed and connected to existing work neatly, safely and professionally. Disturbed or damaged work shall be replaced or repaired to its prior conditions. 8.5.9 In addition to the requirements of the NEC, install an identification sign which clearly indicates information required for use and maintenance of items such as switchboards and panel boards, fused and un-fused safety switches, separately enclosed circuit breakers, and individual breakers. 8.5.10 All items furnished shall be free from defects that would adversely affect the performance, maintainability and appearance of individual components and overall assembly. 8.6.11 Where the Government determines that the vendor has installed equipment not conveniently accessible for operation and maintenance, equipment shall be removed and reinstalled or remedial action performed as directed at no additional cost to the Government. 8.6.12 All information generated in performance of this contract becomes property of the Government and must be surrendered upon request and/or completion of the contract. The Contractor must protect against the destruction and/or misplacement of all information produced under this contract, unless directed under local policy to destroy. This includes both hard and soft copies of data. 8.7. The Contractor shall not release any information (including photographs, files, public announcements, statements, denials, or confirmations) on any part of the subject matter of this contract or any phase of any program hereunder without the prior written approval of the COR. 8.8. Standards of Conduct 8.8.1. Conduct of Personnel. Contractor personnel shall conduct themselves in a professional manner (i.e. timeliness, communication; spoken and written etc.). The Contracting Officer may require the Contractor to remove from the job site any employee working under this contract for reasons of suspected misconduct, a suspected security breach, or suspected to be under the influence of alcohol, drugs, or any other incapacitating agent. Contractor employees shall be subject to dismissal from the premises upon determination by the Contracting Officer that such action is necessary in the interests of the Government. The hospital director or designated representative has the authority to bar individuals from the Government facility. The removal from the job site or dismissal from the premises shall not relieve the Contractor of the requirement to provide sufficient personnel to perform the services as required by this performance work statement. The Government shall not reimburse the Contractor for travel and other expenses associated with the removal of personnel. 8.8.2. Contractor personnel are not authorized to carry or possess personal weapons to include, but not limited to, firearms and knives with a blade length in excess of three inches, while assigned under this contract. 8.8.3. Working Attire and Appearance. Contractor personnel shall present a professional appearance commensurate with standards delineated for Government civilian personnel acting in similar capacities. 8.8.4 The Government reserves the right to review Contractor personnel qualifications. Upon request, resumes shall be provided to the Contracting Officer prior to clearance approval, for review and acceptance by the COR. The intent is verification purpose and not for the Government to become the hiring authority. 9. TRAVEL. NOT APPLICABLE. 10. NON-PERSONAL SERVICE STATEMENT. 10.1. Contractor employees performing services under this order will be controlled, directed, and supervised at all times by management personnel of the contractor. Contractor management will ensure that employees properly comply with the performance work standards outlined in the PWS. Contractor employees will perform their duties independent of, and without the supervision of, any Government official. The tasks, duties, and responsibilities set forth in the contract may not be interpreted or implemented in any manner that results in any contractor employee creating or modifying Federal policy, obligating the appropriated funds of the United States Government, overseeing the work of Federal employees, providing direct personal services to any Federal employee, or otherwise violating the prohibitions set forth in Parts 7.5 and 37.1 of the Federal Acquisition Regulation (FAR). The Government will control access to the facility and will perform the inspection and acceptance of the completed work. 11. PERSONNEL QUALIFICATIONS. 11.1.
