Page 11 of 11 Evaluation Factors The contractor shall be competent & proficient in full repairs and refurbishments to entail complete reinsulating, laser welding, straightening, testing, realigning, machining, powder coating, sharpening, and complete rebuilds as well as other actions needed to bring instruments to original design specification & in compliance with ANSI-AAMI ST-79 part 7.7. The contractor shall furnish evidence of ISO 9001:2015 certified Other Electronic & Precision Equipment Repair & Maintenance certification. Contractor shall provide descriptive response of how you are able to provide 1-hour response time via a phone call and 24 hours onsite. Describe dispatch plan to have service provider on-site to meet an Emergency turnaround time of 24 hours for surgical equipment & sets. Describe your plan to have Extensive repairs completed within a time of three weeks. Provide a narrative plan of how you plan to meet Cart repairs including case carts, loading carts, once per month (infrequent typically 1-2 per year.) Provide a plan on bi-annual autoclaving cleaning. Dayton Tool Sharpening Questions and Answers: Per your answers from solicitation #36C25024Q0079, you stated this was strictly onsite stainless instrument repair and maintenance with lap instrumenting with very little case cart repair and semiannual sterilizer cleaning, but the evaluation factors are identical to the previous solicitant #36C25024Q0079 and referencing rigid scope camera repair and turnaround times. Do you expect evaluation factor #5 to answer pertaining to surgical device repair or should it refer to extensive stainless hand instrumentation sent into the repair depot for repair and the timeframe of completion for those items? All ridged scopes were removed from inventory. The majority of repairs will be surgical stainless steel and Laparoscopic instrumentation. Evaluation factor number 5 reflects this now by the removal of equipment that was listed in parenthesis in initial posting. In our previous set of questions, we asked for clari cation regarding factor #3. You answered the item was being amended to respond within 1 business day to the initial call and then what the response would be for an emergency repair. Can we have this reworded, so it conveys what you are needing answered please? To clarify we will be sticking with an initial phone call response within 1 hour and 24 hours on-site.
