- RFQ HAS BEEN CANCELED as of 11/27/2023 0803 CST Questions: 1. Can you please tell me what is the range of distance needed from patients homes to the hospital? Answer:� �I would say a rough average would be not more than 50 miles. With that being said, there are many that are inside of 10 miles but it is not uncommon for us to have to transport Veterans longer distances (100 miles or more) for specialty care or admittance/discharges from contracted nursing� 2. And, are the drivers required to wait for the veterans to complete their appointments before they can depart? Answer:� The drivers are NOT . required to wait until the Veteran is completed with their appointment. We schedule by the trip and a trip is one way. If there is a need for a return trip or round trip provided by the vendor, then that will be scheduled separately as another trip giving the vendor an idea or some time to possibly complete other trips in between. Employee Assistance Program (EAP) Services for VA Saint Cloud Iowa Health Care System. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This is a solicitation conducted using FAR Part 13 (Simplified Acquisition Procedures). This announcement constitutes the only solicitation, quotes are being requested. Solicitation number 36C26324Q0058 is issued as a request for quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2023-04. This procurement is a total set-aside for Service-Disabled Veteran Owned Small Business (SDVOSB). Prospective offerors must be registered and current in the System for Award Management (SAM) database at time of offer to the Government (https://www.sam.gov). Quotes will only be accepted from responsible contractors. The NAICS code for the solicitation is 485991 with a size standard of $19 million. A list of the contract line items, quantities, and units of measure are listed within the attached solicitation documentation. The VA anticipates one firm fixed price contract award to provide services to VA Saint Cloud, Minnesota Health Care System, 4801 Veterans Avenue, St. Cloud, MN 56303 Health Care System. The period of performance is 12-01-2023 to 11-30-2024 for the base year period of performance. There are four (4) one-year option periods available at the government s discretion. FAR provisions 52.212-1, 52.212-2, and 52.212-3 apply to this procurement and are detailed within the attached solicitation document. FAR clauses 52.212-4 and 52.212-5 apply to this procurement and are included within the attached solicitation document. Additional requirements are listed within the attached solicitation document. The Defense Priorities and Allocations System (DPAS) does not apply to this acquisition. Technical questions on this solicitation are due via e-mail only to Becky De Los Santos, Contracting Specialist, at Becky.Delossantos @va.gov no later than 10:00am Central Standard Time (CST), 11-20-2023. Quotes are due to the Contracting Office by 10:00am CST, 11-27-2023. Quotes are required to be emailed to Becky.delossantos@va.gov prior to the solicitation closing date and time.
