This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-01. This solicitation is set-aside for SDVOSB. The associated North American Industrial Classification System (NAICS) code for this procurement is 339113. The FSC/PSC is 6515. The NCO 10 for Cleveland VA Medical Center is seeking to purchase the SMR MAXI CABINET for the ENT Clinic. All interested companies shall provide quotations for the following request. STATEMENT OF WORK The requested twelve (12) SMR Cabinets are for the replacement of equipment that is end of life and in coordination with the expansion of clinical areas (ENT Module E/F) within the Medical Center and Ravenna Outpatient Clinic. The requested solution/equipment shall be interoperable with current workflows/needs within the mentioned clinical areas and be consistent with SMR Cabinets refreshed at other clinic locations. These SMR Cabinets will be utilized in different clinical areas with varying needs. The requested equipment can meet the clinical applications in all locations without any needed alterations for use. 1.0 GENERAL SCOPE The Veterans Health Administration (VHA) has a requirement for SMR Cabinets for replacement and expansion of clinical space located at Cleveland VA Medical Center. The requested equipment solution shall provide the replacement and addition of the above listed equipment in accordance with the listed salient characteristics provided under the scope to meet the requirement in full. The contractor is responsible for providing the delivery, removal, and installation of all equipment and coordinate the shipment/delivery to the locations listed above within sufficient time to allow installation in each exam room. All installation services shall be provided through the original manufacturer of the requested SMR Cabinets to the clinic locations and shall be responsible for setup/installation by a fully trained/certified OEM representative(s), and finally, removal of the old units being replaced. The requested services shall be performed in accordance with the OEM s premium white glove delivery/install services. These services shall include, but may not be limited to, the following: A single point of contact for all project coordination Trained team from the OEM shall provide delivery and setup for all equipment and associated accessories. Delivery and setup service shall include delivery, uncrating, trash removal, product and accessory setup, and placement of units Insurance coverage shall be provided for full delivery and setup period Manufacturer recommended operational test shall be performed and provide appropriate outcomes 2.0 PERFORMANCE REQUIREMENTS The vendor will deliver the cabinets in its original package to the Cleveland VA warehouse. From there, warehouse and engineering will coordinate the delivery and installation of the cabinets to room 2-B309A. The requested equipment shall be consistent with recent acquisitions of SMR Cabinets within the Medical Center and surrounding Outpatient Clinics. This equipment (or equivalent) family of SMR Cabinets are being requested for consistency and performance. The SMR Cabinets shall be purchased with appropriate colored tops to coordinate with Interior Design specifications, and to be fully compatible for use within the clinical area and fit in the designated space. It is requested that the vendor be capable of meeting the expedited delivery of these Cabinets to ensure that the Clinical space can be opened in accordance with the activation timeline of January 20, 2024.
