Description
REQUEST FOR INFORMATION (RFI) INSTRUCTIONS: The Department of Veterans Affairs, Veterans Health Administration (VHA), Network Contracting Office 08 (NCO 08) is conducting a market survey and is seeking potential sources for a for a complete removal and complete replacement for a new Physiological Monitoring & Telemetry System referenced Statement of Need (SON) provided below BRAND NAME OR EQUAL. This Sources Sought Notice is issued for information and planning purposes only. This is not a solicitation or a request for proposal and shall not be construed as an obligation or commitment by the Government. An award will not be made on any offers submitted in response to this notice, and it shall not be implied the Government is committed to providing any solicitation or award following this notice. The Government will not pay for any information received in response to this request, nor will the Government compensate a respondent for any costs incurred in developing the information provided. Any contract that might be awarded based on information received or derived from this market research will be the outcome of a competitive process. Interested parties are responsible for adequately marking proprietary, restricted or competition sensitive information contained in their response. Responses shall be submitted to janice.fornaro@va.gov by 1:00 p.m. Eastern Standard Time (EST) on Thursday, December 21, 2023. This notice is intended strictly for market research. The purpose of this Sources Sought Notice is to determine interest and capability of potential qualified sources of supply and determine the socioeconomic size classification of the supplier and manufacturer of the end items. Interested companies shall complete, at a minimum, the following information with their response: The information identified below (the Statement of Need) is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications mentioned in the Statement of Need below. Responses to this RFI should include: (1) Company name, address, (2) Point of contact, (3) Phone number, (4) Point of contact e-mail, (5) UEI Number, (6) DUNS Number, (7) Cage Code, (8) Size of business pursuant to North American Industrial Classification System (NAICS) 334510 Electromedical and Electrotherapeutic Apparatus Manufacturing (size standard of 1,250 employees). Please answer the following questions: Please indicate the size status and representations of your business, such as but not limited to: Service-Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Hubzone, Woman Owned Small Business (WOSB), Large Business, etc.). Is your company considered small under the NAICS code identified under this RFI? If you are not the original equipment manufacturer (OEM), can you comply with the nonmanufacturer rule In accordance with 13 CFR 121.406(b)? If your company is an SDVOSB or VOSB, would your company be compliant with VAAR 852.219-76, and would you sign the VAAR Clause 852.219-76 and return with your offer during a solicitation? Can your company meet FAR 52.219-14 Limitations on Subcontracting? What percentage of the contracted products/services can your company complete as the prime contractor? ___________% What percentage of the contracted products/services would your company subcontract? ____________% If you intend to subcontract any work on this contract, please articulate what work you will be doing as the prime and what work the subcontractor, if any, will be doing. Would the companies receiving subcontracts be like situated companies? This means, if your company is certified SDVOSB, would another SDVOSB receive the subcontracts. Likewise, for VOSB s, etc. (CFR § 125.1 and § 121.103(h)(3)). If you are a large business, do you have any designated distributors? If so, please provide their company name, telephone, point of contact and size status. If you re a small business and you are an authorized distributor/reseller for the items identified above or an equivalent solution, do you alter; assemble; modify; the items requested in any way? If you do, state how and what is altered; assembled; modified? If you are an authorized distributor for the OEM, please provide an authorized distributor letter. Does your company have an FSS contract with GSA the NAC, SEWP or are you a contract holder with any other federal contract? If so, please provide the contract number and the ultimate expiration date of the contract (must be able to be in effect during the duration of the period of performance stated in the statement of need). If you are an FSS GSA/NAC/SEWP contract holder or other federal contract holder, and intend to utilize your Government-Wide Acquisition Contract (GWAC) for this requirement, are all the items/solutions you are providing information for available on your schedule/contract, not that of the OEM s (as awards cannot be made outside of the awardee s FSS/NAC/SEWP or GWAC contract not containing the type of products or services the agency set out in its RFQ an agency cannot lawfully use GWAC ordering procedures to order products/services not included on a vendor s schedule contract.)? If you are not the OEM, and anticipating utilizing a GWAC, please provide your GWAC schedule for review. Your GWAC schedule must have all of the products/services available upon responding to a potential upcoming solicitation and open market items cannot exceed the micro-purchase threshold. If you are an FSS GSA/NAC/SEWP contract holder or other federal contract holder what is the approximate dollar amount would be provided via the open market? What is the manufacturing county of origin for your products? Does the patient Monitoring System that would be offered meet the statement of need, below? Please submit your capabilities addressing your ability to meet the requirements in regard to the statement of need and any information pertaining to equal to items and describe your company s general experience providing complete system replacements to government or to commercial customers withing the past three years to establish capabilities for planning purposes (no longer than 10 pages). Please review salient characteristics/statement of need (if applicable) and provide feedback or suggestions. If none, please reply as N/A. This RFI will be conducted in accordance with Federal Acquisition Regulation (FAR) Part 15. Telephone responses will not be accepted. Responses must be received via e-mail to janice.fornaro@va.gov no later than, 1:00 PM Eastern Time (ET) on December 21, 2023, using the RFI number 36C24824Q0300 in the subject line. This notice will help the VA in determining available potential sources only. Do not contact VA Medical Center staff regarding this requirement, as they are not authorized to discuss this matter related to this procurement action. All firms responding to this RFI are advised that their response is not a request for proposal, therefore will not be considered for a contract award. If a solicitation is issued, information will be posted on the SAM.gov website for all qualified interested parties at a later date and interested parties must respond to the solicitation to be considered for award. This notice does not commit the government to contract for any supplies or services. The government will not pay for any information or administrative cost incurred in response to this RFI. Information will only be accepted in writing by e-mail to Contracting Officer at janice.fornaro@va.gov and include 36C24824Q0300 in the subject line.
