- INTENT TO SOLE SOURCE GENERAL INFORMATION Network Contracting Office (NCO) 22 Attn: Barry Bland Department of Veteran Affairs Network Contracting Office 22 4811 Airport Plaza Drive Suite 600 Long Beach, CA 90815 (858) 626-8744 Intent to Sole Source: The Government intends to make a sole award firm-fixed-price contract in accordance with Federal Acquisition Regulations (FAR) 6.302-2 Only One Source in conjunction with FAR 13.5 Simplified Procedures for Certain Commercial Items. The requirement is for emergency sewage leak in the emergency department and 2C ward, necessitating immediate action for abatement, repairs, accessibility improvement, and implementation of preventive measures at Phoenix VA Healthcare System - 650 E Indian School Rd, Phoenix, AZ 85012. Scope of Work - Emergency Abatement: Secure the affected areas to prevent unauthorized access by building barriers and providing harmful air as necessary. Remove contaminated water and materials from the following Bldg.1 rooms C229, C229, C225, C220, C221, C228, C226, C219, CC4B, 2C IT closet, C208A, 2C Nurse s station inside and out, C136, C136A, C133, C133A, C121, C123, C132A, C131, hall in front of C133 and nurse s station. Also, any ceiling tiles throughout the affected areas: Dispose of hazardous waste by local regulations; Clean and disinfect all affected surfaces; and Conduct air quality testing to ensure a safe environment. Repairs: Structural Repairs - Repair and reinforce any structural damage caused by the sewage leak, including replacing drywall, cove base, corner molding, ceiling tiles, flooring, etc. Plumbing Preventive Measures: Implement measures to prevent similar incidents in the future. Repair damaged plumbing and install new accessible cleanouts for future incidents. Replace 5-floor drain connections in the center portion of 2C (C225, C224A) that connect to the 4 central with new epoxy piping. Replace 4 main stack sewer piping with epoxy-coded connections at the Y and 45s leading to the horizontal main. This will require a water outage and must be completed between 10:00 pm and 6:00 am. Add plumbing cleanout in the wall between C224 and C224A. This will require a water outage and must be completed between 10:00 pm and 5:00 am. Replace Spud gaskets and Slone valve water stops on toilets in the following rooms (C233, C231A, C227A, C225A, C220A, C219A, C218A, C217A, C216A, C215A, C214A, C210, C211A, C206A, C203A, C200A). Provide high-pressure jetting of 4 /6 main drain for the entire C tower. The intended source is Phoenix VA Healthcare System. In accordance with FAR Part 6, contracts awarded using this authority shall be supported by a written justification and approval described in FAR 6.303 and 6.304; as applicable, the justification will be posted within fourteen (14) days after contract award. The Government will consider all capability statements received by this synopsis's closing date. The Government's determination not to compete based on responses to this notice is solely within the Government's discretion. Information received will be considered strictly to determine whether to conduct a competitive procurement. The applicable North American Industrial Classification System Code (NAICS) code that this work falls under is 237110 - Water and Sewer Line and Related Structures Construction, with a size standard of $45 Million. THIS NOTICE OF INTENT IS NOT A REQUEST FOR COMPETITIVE QUOTATIONS. All capable sources must provide past performance experience in similar research services and be a licensed provider (whichever applies to the requirement). All vendors must be registered in Beta. SAM by the close of this notice, and if your firm is a Service-Disabled Veteran- Owned or Veteran-Owned Small Business, you must be CERTIFIED in VetCert (see internet site: Veteran Small Business Certification (sba.gov.)). Proof of licenses must be submitted with supporting evidence. All the above information should be included in the vendors' capability in response to this notice.
