Description
Oklahoma City Veterans Affairs Health Care System (OKC VA HCS) Statement of Work (SOW) Replace Flooring, Dish Room 2B-102m GENERAL: This project is to replace flooring in Dish Room, 2B-102m at the OKC VA HCS at Oklahoma City, Oklahoma. The contractor is fully responsible for verifying and validating all information contained within this statement of work. Contractor must fully evaluate the condition of the existing structures and verify the current condition of the project site. All measurements/quantities provided in this document are approximate and are to be used for reference only. The contractor will be responsible for taking on-site measurements and verifying all quantities and distances. All materials/fasteners/etc. must be new and provided by the contractor unless otherwise specified. The contractor shall abide by all Safety, Security and Infection Control Policies and Procedures of the Oklahoma City VA HCS. VA facilities are tobacco free. No smoking is permitted on VA property. The contractor is responsible for their subcontractor s compliance with this policy. All work must conform to, and comply with, the latest edition/version of ALL applicable VA, federal, state, and local laws, rules, regulations, codes, and standards. JUSTIFICATION: Existing flooring has numerous amounts of cracks which is causing leaks to reach underneath the existing tile flooring. Replacing the tile flooring will alleviate any leak issues. STATEMENT OF WORK: The contractor shall provide supervision, manpower, and materials to remove existing tile flooring and install new flooring tile. Contractor shall remove existing tile flooring & cove base. Contractor shall prepare cove base & floor for new flooring as per flooring manufacturer installation instructions. Contractor shall apply Laticrete Hydroban waterproofing and crack isolation. Contractor shall utilize epoxy grout to match nearby existing color. Contractor shall confirm with CO/COR style color of cove base (5x6 Round Top Base (Q3565F1P) Contractor shall confirm with CO/COR style/color of flooring tiles to be installed (Dal Tile Quarry Tile 0Q40 Red Blaze 6x6 (66F1P) Contractor shall relocate dishwasher equipment, pulper and tabling to allow full flooring access. Contractor shall relocate dishwasher equipment, pulper and tabling to location determined by the COR. Contractor shall return dishwasher equipment pulper and tabling to original location and ensure equipment is properly secured and functional. Contractor shall dispose any trash or debris they generate during the duration of the performance period and completion of project. Clean up area after project completion and contact COR for final inspection. PHOTO REFERENCES: Dish Room WORK SCHEDULE Normal working hours are from 7:30 AM to 4:30 PM (local time) Monday through Friday except for weekends and established Federal Holidays. Location of Work All work associated with this project will be completed at the Oklahoma City VA Health Care System, located at 921 NE 13th Street, Oklahoma City, OK 73104. Period of Performance This contract is expected to start upon Notice to Proceed (NTP) and be completed within 30 calendar days of issuance.
