This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-06 Effective date 12/04/2023. This solicitation is set-aside for SDVOSB The associated North American Industrial Classification System (NAICS) code for this procurement is 811310, with a small business size standard of $12.5 M. The FSC/PSC is J066. The STERILE PROCESSING SERVICE is seeking to purchase Surgical Instrument Repair Service. Place of Performance/Place of Delivery Address: 1601 S.W. Archer Rd. Gainesville, FL Postal Code: 32608 Country: UNITED STATES The following solicitation provisions apply to this acquisition: FAR 52.212-1, Instructions to Offerors Commercial Products and Commercial Services (NOV 2021) Offerors are to submit any questions regarding this requirement via email to jose.hernandez@va.gov on or before 3:00 pm on 1/30/2024 with the subject line, Surgical Instrument Repair Service. An amendment responding to all questions, which may or not extend the response due date, will be published at the GPE. Offerors shall e-mail complete quotes to jose.hernandez@va.gov at the date and time stated on the Combined Synopsis/Solicitation Notice and Description. Provide the following information: 1. Solicitation number 2. Name and address of your company 3. Point of contact; name, e-mail, and phone number 4. UEI number 5. Unit and Total Prices as specified in Section Price Schedule. Note: To be considered for award, an offeror shall have an active registration in the System for Award Management (SAM). Offerors must complete annual representations and certifications electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal in accordance with FAR 52.212-3, Offerors Representations and Certifications Commercial Items. If paragraph (j) of the provision is applicable, a written submission is required. All quoters shall submit the following: Detailed quote with each line item priced out. Proof Technician or staff are certified by International Association of Healthcare Central Services Materials Management (IAHCSMM) or the Certification Board for Sterile Processing and Distribution (CBSPD). Instrument Technician Training Program must be approved through the Department of Labor. Provide information and verification of offsite Lab. All quotes shall be sent to the NCO 8, jose.hernandez@va.gov Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. Submission of your response shall be received not later than 3pm ET, February 9, 2024 at jose.hernandez@va.gov. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f).
