Description

The purpose of the attached amendment 0005 is to�provide responses to final questions received. The purpose of the attached amendment 0004 is the following: A Provide Updated Statement of Work reflecting revision of a 90 day transition period. B. Provide responses to follow up questions. The purpose of amendment 0003 is the following: A. Extend the offer submission date to Monday, February 5th, 2024. B.� Provide Updated Addendum 52.212-1, noting follow up questions are due by Thursday, January 18th, 2024, at 17:00 PST. C. Provide Updated Statement of Work. D. Provide Updated Contract Line Items. E. Provide responses to remaining questions. The purpose of amendment 0002 is the following: A. Provide answers and clarifications in attachment 01, 36C26124Q0072 0002; SNVHCS Las Vegas Home Oxygen Services and Supplies Questions, to clarifications requested and questions received. B. Extend the offers due date from January 12th, 2024 to January 22nd, 2024 to allow time for revisions to the statement of work, request for quote, and further required clarifications. The purpose of the attached amendment 0001 is to extend the offers due date from Friday, December 29, 2023 at 1700 PST toJanuary 12th, 2024 at 15:00 pm PST due to pending questions received.
