All information received in response to this notice is marked as proprietary and will be handled accordingly. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this notice. This notice is being issued in an attempt to determine the level of interest among all socioeconomic groups. Responses will assist VA planners in determining an appropriate acquisition and set-aside strategy which best fits VA requirements. The James E. Van Zandt VA Medical Center (hereafter referred to as JEVZ VAMC , Altoona VAMC or the Government ) is performing market research to determine if there is a sufficient number of qualified: (1) Service-Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); (3) Small or emerging small business firms (SB or other); or (4) large business (LB) firms to set aside a single award contract to a qualified contractor to provide post-warranty service on audiology testing devices and booths located at the Altoona VAMC as well as the Johnstown, DuBois, Huntingdon, Indiana, and State College Community Based Outpatient Clinics (CBOC). If applicable, SDVOSB and VOSB must be verified in SBA at Veteran Small Business Certification (sba.gov). The JEVZ VAMC is seeking a five-year, IDIQ firm-fixed priced contract to offer preventative maintenance and repair Services. Attached is the Draft Statement of Work (SOW) for a description of this requirement. The anticipated period of performance is to start May 1, 2024, for a total of five years. The North American Industry Classification System (NAICS) code being considered for this procurement is 811210. All interested firms who can meet the requirement as outlined in the Draft SOW, should respond in writing, and provide the following information along with their capability statement: 1. General a. Explain your Type of Business: Type of Business: Veteran Owned; Small; Small Disadvantaged; or Large, etc. b. If you are considered a large business under NAICS 811210, are you familiar with subcontracting plans and goals? c. If you are a small business under NAICS code 811210, are you familiar with the limitations on subcontracting in FAR 52.219-14? 2. Describe your firm s capability to perform the tasks as outlined in the SOW and include: a. Company name and address. b. Please describe your experience and qualifications as it directly relates to this requirement. (Education, apprenticeships, past contracts, certifications ) c. List the days/hours of operation. d. If registered in System for Award Management (SAM), provide the Unique Entity Identifier (UEI). e. List Point of Contact information (title, phone number, and email address) f. Are you planning on subcontracting, if so, what part(s) of the SOW? g. Describe any additional relevant information. (List any qualifications/certifications) 3. Existing Contracting Vehicles: Do you have a GSA FSS contract or other applicable contract vehicle if within scope of this effort (if applicable)? If so, indicate contract number, SIN, expiration date, etc. and include how your existing contract is within scope of this requirement. 4. Are you currently enrolled SAM? SAM.gov | Home Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications required in the Systems for Award Management (SAM) database prior to award of a contract.
