- ***NOTICE OF INTENT TO SOLE SOURCE*** Synopsis: The Department of Veterans Affairs; RPO-E, Network Contracting Office (NCO) 7, located at 501 Greene Street, Hatcher Bldg. Ste 2, Augusta, GA 30901 intends to award a firm-fixed price sole source requirement to: Four Points Technology, LLC. 13221 Woodland Park RD STE 40 Herndon, Virginia 2171-5505 Program Need Date 06/01/2024 est. The applicable NAICS code for this procurement is 541519 with a business size standard of $34 million. The proposed contract is for Phillips Xper Flex Cardio / Hemodynamic Monitors for which the Government intends to solicit with a single source award to a Service-Disabled Owned Veteran Owned Small Business (SDVOSB) GWAC in accordance with FAR 16.505. This notice of intent to sole source is not a request for quotes; however, all responsible sources may submit a capability statement which shall be considered by the Government. All capability statements must be received no later than 08:00 AM EST on Friday, April 05, 2024. Responses are to be submitted to Joseph E. Horn, Contracting Officer at Joseph.Horn@va.gov . A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. This notice of intent is NOT a request for competitive proposals. If you have questions, please contact Contracting Officer Joseph Horn at Joseph.Horn@va.gov . STATEMENT OF WORK GENERAL INFORMATION Purchase: Xper Flex Cardio Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. Contractor shall provide Xper Flex Cardio. Work shall meet all applicable codes and regulations and is subject to the terms and conditions of the contract. Background: Cardiology needs the equipment for improved clinical decision making and improved access to care. Sensitive data will rest on the device. Performance Period and Delivery Schedule: The contractor shall provide the specific deliverables described below within the performance period stated in this SOW, unless otherwise directed by the Contracting Officer (CO). Delivery will be to the Atlanta VA MC 1670 Clairmont Rd. Decatur GA 30033 (Logistics). Type of Contract: Firm-Fixed-Price GENERAL REQUIREMENTS For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task, and along with associated sub-milestone dates. The contractor's subtask structure shall be reflected in the technical proposal and detailed work plan. All written deliverables shall be phrased in layperson language. Statistical and other technical terminology shall not be used without providing a glossary of terms. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES `Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described below within the performance period stated in Section A.4 of this SOW. Task One: The contractor shall provide an exact match to the listed product needed within 30 days. Deliverable One: Delivery is by a third-party entity i.e., USPS, UPS, FedEx, etc. to be shipped to: Atlanta VA Medical Center c/o Logistics 1670 Clairmont Road Decatur GA 30033 Task Two: The delivery schedule will be met; unless requested by the Contracting Officer, COR, or Logistics Inventory Specialist. Deliverable Two: The contractor will agree to the delivery schedule. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. Salient Characteristics: Improved lab communication Enables continuous patient monitoring Improved radial access workflow Enhanced workflow through integrated iFR functionality The NAICS Code for this requirement is 541519 - Other Computer Related Services and the size standard is $34 million.
