- Amendment 36C25623R0079 0005 has been posted to provide the answers to all follow-up questions received after the submission of Amendment 0003. Amendment 36C25623R0079 0004 has been posted to revise the due date for proposals from 15 April 2024 at 3PM CST to 15 May 2024 at 3PM CST. The amendment also answers follow-up questions and amendment 0005 will answer additional follow-up questions. Please see the posted amendment attachment 36C25623R0079 0003. Amendment 36C25623R0079 0003 has been posted to provide the answers to all questions received by the deadline of of 1 March 2024 at 3PM CDT. Please see the posted amendment attachment 36C25623R0079 0003. Amendment 36C25623R0079 0002 has been posted to�extend the due date for proposals from 29 March 2024 at 3PM CST to 15 April 2024 at 3PM CST. The extended due date for proposals will allow for additional time to answer all questions received and additional time to post the amendment to provide all answers to questions. This amendment also changes the date proposals are due at E.2 Addendum to FAR 52.212-1 from 29 March 2024 at 3PM CST to 15 April 2024 at 3PM CST. The extended due date for proposals will allow for additional time to answer all questions received and additional time to post the amendment to provide all answers to questions. This amendment also changes the date proposals are due at E.2 Addendum to FAR 52.212-1 from 15 March 2024 at 3PM CST to 29 March 2024 at 3PM CST. SERVICES REQUIRED: The Veterans Health Care System of the Ozarks requires the following services to be provided in a private hospital, office or clinic environment to Veterans, primarily residing in Harrison, Arkansas. Primary Care Community Outpatient Clinic (CBOC): offers both medical and mental health support services (physically on site and by telehealth) and may offer support services such as pharmacy, laboratory, and x-ray. Primary Care CBOCs are required to provide both primary care and mental health support services. Note*** Mental Health services will be provided by the Veterans Health Care System of the Ozarks. The contractor will be required to provide mental health registered nurse support staff and space for the VA Mental Health providers. There will be no line item for mental health services as only support services will be required. PLACE OF PERFORMANCE: The Contractor shall provide a community-based outpatient clinic (CBOC) within the city limits of Harrison, Arkansas. AUTHORITY: In accordance with Title 38 United States Code (USC) 8153 to be furnished by the contractor on behalf of the Veterans Health Care System of the Ozarks. Procurement Authority is in accordance with FAR Parts 12 and 15.
