Description
Statement of Work Handicare Patient Lifts MARTINSBURG VA MEDICAL CENTER 2237: 613-24-3-034-0181 INTRODUCTION The purpose of this requirement is to provide full-service maintenance and repair for 175 Handicare Patient Lift C-Series, Models: C1000 and C625, and associated accessories at the Martinsburg VA Medical Center. This requirement includes the scheduled maintenance and corrective maintenance according to the manufacturer s recommendations/procedures. The contract shall include all parts, materials, labor, and travel to complete servicing of the Handicare C1000 and C625 patient ceiling lifts. Locations: Martinsburg VA Medical Center, 510 Butler Ave, Martinsburg, WV 25405 Winchester VA Clinic, 100 Dawson Drive, Winchester, VA 22602 PERIOD OF PERFORMANCE The base period of performance (POP) for this contract shall begin on July 1, 2024, unless otherwise specified, and shall expire on June 30, 2025. This solicitation includes the provision of quotations for this base year, and four (4) subsequent option years beginning July 1, 2025, that may be exercised at the discretion of the government. Base Year July 1, 2024 June 30, 2025 1st Option Period July 1, 2025 June 30, 2026 2nd Option Period July 1, 2026 June 30, 2027 3rd Option Period July 1, 2027 June 30, 2028 4th Option Period July 1, 2028 June 30, 2029 WORK HOURS Normal hours of coverage are Monday through Friday from 8:00 a.m. to 4:30 p.m., excluding the federal holidays listed as follows: Federal Holidays: New Year s Day Dr. Martin Luther King Jr s Birthday Presidents Day Memorial Day Juneteenth Independence Day Labor Day Columbus Day Veteran s Day Thanksgiving Day Christmas Day Any other day specifically declared a National Holiday by the President of the United States When one of the above designated legal holidays falls on a Sunday, the following Monday will be observed as a legal holiday. When a legal holiday falls on a Saturday, the preceding Friday is observed as a holiday by US Government Agencies. EQUIPMENT Equipment for service will include 175 Handicare Patient Ceiling Lifts, Models: C1000 and C625. SPECIFICATIONS Preventative Maintenance Preventative maintenance shall be completed within the first 60 days from contract award. The contract shall include all preventative maintenance of the equipment and accessories to include rails, trollies, gates, transitions, and carousels according to manufacturer s recommendations/procedures in the equipment manual. All tasks in the manufacturer s procedures shall be completed at the manufacturer s listed frequencies. All lift batteries shall be replaced by the contractor during Preventative Maintenance (PM) inspection on odd numbered years. All track, trolly and transition bearings shall be replaced by the contractor during PM inspection on even numbered years. Load Testing - Contractor will guarantee proper functional load testing specification (for all included equipment) will meet manufacturer preventative maintenance standards for one full year. Contractor shall provide certificate of load testing for the equipment. Contractor shall provide calibration certificates of any testing equipment utilized. Corrective Maintenance The contract shall include all corrective (unplanned) maintenance/repair of the equipment in accordance with the manufacturer s manual. Recalls & Alerts-- Contractor shall perform all remediation actions needed for product alerts and recalls issued for the equipment. Parts/Materials/Tools/Labor The contractor shall provide small parts needed to complete maintenance that could be reasonably foreseen as needed due to standard wear and tear, i.e. track, trolly, and transition bearings, internal air lines and gear lubricant. The facility shall provide lift batteries when a battery change is required. Contractor shall provide their own tools to complete the maintenance. All Test Equipment utilized must meet NIST and ISO 9001 standards. Technicians must provide proof of calibration of instrumentation upon before or upon arrival. All invoices must contain the model, serial number and calibration date/due of test equipment utilized. The C&A Requirements do not apply and that a Security Accreditation Package is not required. All labor (including travel) to complete the maintenance is included in the contract. Check in/Out Contractor personnel arriving on-site to perform work shall report to the Biomedical Engineering Department. Contractor personnel shall sign in using the department s vendor sign in sheet, filling out all required information. Contractor personnel s tool bags are subject to inspection by the Biomedical Engineering department upon arrival and upon departure. The contractor shall be escorted to the equipment by a Biomedical Engineering department designee. Contractor personnel shall sign out upon completion of visit. Prior to departure, Contractor personnel are required to, at a minimum, verbally report what work was performed and any issues with the equipment to the Biomedical Engineering department, especially noting if the equipment is still down or not fully repaired. The VA campus is non-smoking. Contractor personnel are required to comply with this policy. Parking. It is the responsibility of the Contractor to park in the appropriate desig nated park ing areas. Reporting Upon award, the Contractor shall designate to the COR, in writing, the name and telephone number of the point of contact for all actions on this requirement. Contractor shall provide a copy of the service report, complete with test data, to the contract COR within five working days after completion of any services. Service reports shall include identifying information about the equipment, all services provided during the service visit, any issues found during the visit, the technician/engineer names of the personnel performing work, and the date service was provided. Notification needs to be given to the COR of any parts/equipment being replaced. Contractor Requirements All Contractor personnel performing services on the equipment shall have factory training and experience in the maintenance and repair of patient ceiling lifts in a medical facility, i.e. Industrial setting. Contractor shall be able to provide three (3) or more examples of previous patient ceiling lift preventative/corrective maintenance performed at VA medical center facilities. Contractor shall provide copies of training certificates for the Contractor personnel performing work on-site upon the request of the Government. Contractor shall use the VA Program Office Standardized Corrective and Preventative Maintenance Checklist for Ceiling Mounted Patient Lifts (Attached) as a baseline standard when completing any preventative or corrective services. These will be provided by the facility POC, and the contractor shall document findings per ceiling lift on this checklist. The contractor's personnel shall be able to read, write, understand and speak English clearly, to effectively communicate with patients and/or other healthcare providers and staff. Safety and Security Contractor shall follow all Occupational Safety and Health Administration (OSHA) laws and regulations. The Contractor is responsible for reporting any hazards they come across to their point of contact. Contractor shall follow all local procedures concerning infection control. Should the Contractor be unsure of the requirements, the COR can provide the specific information and procedures. Contractor personnel shall wear visible identification at all times while on VA property. Contract personnel are required to identify themselves as such to avoid creating an impression in the minds of members of the public that they are Government officials. Property Damage The contractor shall take all necessary precautions to prevent damage to any Government property. The contractor shall report any damages immediately and shall be assessed current replacement costs for property damaged by the contractor, unless corrective action is taken. Any damaged material (i.e., trees, shrubs, lawn/turf, curbs, gutters, sidewalks, etc.) will be replaced in a timely manner or corrected by the contractor with like materials, at no extra cost to the Government, upon approval of the Contracting Officer. QUALITY ASSURANCE End-users, Biomedical Engineering, and/or the contract COR shall perform system verification tests to ensure the system is fully functional following service/repairs. Issues with the quality of the Contractor s work will be communicated through the Contracting Officer to the Contractor point of contact for resolution. Documentation with be completed in the Government systems. The Contractor is required to follow all mandatory documents to the extent they apply to this contract. Any such changes to mandatory publications that cause a change in the scope of performance will not be implemented by the contractor until the Contracting Officer issues a modification in writing.
