Description
I. General Description: Provide Construction Services for Project#: 695-23-103 to Replace Building 5 Roof. Project Scope: General Description: Building 5: Contractor shall provide for and furnish all labor, materials, equipment, qualified supervision, and other items designated in this contract in accordance with the specifications and drawings to complete Project # 695-23-103: Replace Building 5 Roof at Clement J. Zablocki VA Medical Center, 5000 West National Avenue Milwaukee, WI. The existing roof on building 5 consists of a gambrel roof that connects to the low-slope south porch roof. The gambrel portion is currently clad in asphalt shingles with wood trim and a metal drip edge at the transition between slopes. The low slope South porch roof has an EPDM membrane that carries up under the singles of the gambrel roof. The roof has several deficiencies: Missing shingles. Deteriorated roof decking. Improper flashing around transitions and penetrations. Damaged membrane wrapping an exterior door threshold. Failed sealant. Missing cricket at the north side of the elevator overrun. Deteriorated roof ladder anchor allows water to enter below the roof membrane. The South Second Floor Deck is deteriorating, several deck boards are rotten and are being pulled away from the joists. The deck joist will also need to be replaced, and the 4th Post on the East is deteriorated. This project aims to correct deficiencies in, remove and replace roofing materials and the south porch second-floor decking of Building 5 of the VAMC Milwaukee. Flashing, valleys, gutters, downspouts, casings, stools, and aprons associated with the roof will also be replaced. The existing wood roof surface and subsurface are to have their conditions assessed, with recommendations for replacement where required. Existing structural members are to remain where possible. Appropriate infection control, life safety, and asbestos abatement measures shall be practiced during construction. Penetration repairs, firestopping, and restoration shall be performed in areas affected by construction. Construction work shall not interrupt the building s operations. VA Departments nor VA Employees shall be displaced throughout construction. Construction work shall adhere to phasing and hours of work as specified in the drawings and specifications. All work shall be completed within 255 calendar days after receiving Notice to Proceed. Project Duration/Phases: Phase 1: Pre-construction (kick-off) Meeting is anticipated to be scheduled on the effective date identified in the Notice to Proceed (NTP). Phase 2: Pre-Construction Period A, Accident Prevention Plan and Construction Schedule to be completed no later than NTP + 30 days. Within this period the contractor shall submit the Accident Prevention Plan, submit the Construction Schedule, and schedule a review meeting with the COR and VA Safety. Phase 3: Pre-Construction Period B duration is NTP + 60 calendar days (not to exceed). Within this period the contractor shall submit the following (as applicable per the specification requirements): Construction Site Security Plan- 01 00 00 Construction Phasing Plan- 01 00 00 Existing Conditions Survey- 01 00 00 Contractor s Impact Statement & Checklist- 01 01 10-SN Construction Schedule (Initial)- 01 32 16.15 Submittal Register- 01 33 23 Schedule of Values- 01 33 23 SF 1413s (Subcontractor List)- 01 33 23 Surety Bonds & Certificates of Insurance- 01 33 23 Accident Prevention Plan- 01 35 26 Activity Hazard Analyses- 01 35 26 SSHO and CP Memorandum- 01 35 26 OSHA Certificates and Applicable Training Documents- 01 35 26 Infection Control Plan -01 35 26 Fire Safety Plan- 01 35 26 Construction Waste Management Plan- 01 74 19 Sustainable Construction Spreadsheet- 01 81 13 Phase 4: Pre-Construction VA Review Period. Phase 4 is anticipated to be 30 calendar days and shall be completed no later than NTP + 90 calendar days. Within this period the VA shall complete all review and approvals of contractor Pre-Construction submittals. Once all Pre-Construction Submittals have been approved and the contractor is ready to mobilize, a Construction Kickoff Meeting will be held. Phase 5: Construction Period Phase duration is anticipated to be 120 days; to be completed no later than NTP +210 . All applicable documentation listed in Phases 2 and 3 and any necessary submittals required to mobilize must be approved by the COR prior to commencing work. No demolition or construction shall be started without COR review and approval of those items listed in the Pre-Construction Period unless authorized in writing by the COR or the CO. Should naturally occurring weather delays occur, the contractor shall restart construction immediately once conditions permit. Phase 6: Post-Substantial Completion Period duration is NTP + 255 calendar days. The contractor shall provide all required materials to include the following within 30 days of completion of Phase 5. Inspection and Approval by the COR and/or the CO. Final Approval and Beneficial Occupancy when applicable. Warranties documents and O&M Manuals. As-built drawings and record submittals. Staff training when applicable. Waste Reports. Existing Conditions Re-survey. Release of Claims. The last 15 days of the Post-Substantial Completion Period is reserved for the VA to review and approve the submitted closeout documents, which will be completed no later than Formal NTP + 240 calendar days. -- End SOW
