Page 5 of 5 NOTICE OF INTENT: This is not a request for offers, quotes, or proposals and in no way does this notice represent a commitment by the Government to issue a solicitation or award a contract. Any information provided in response to this notice is solely for market research purposes. The U.S. Department of Veterans Affairs (VA), through the Office of Procurement, Acquisition and Logistics (OPAL), on behalf of the Veterans Health Administration (VHA) and the Strategic Acquisition Center (SAC), is issuing this Request for Information (RFI) in the hopes of attaining sufficient information for market research purposes. The VHA is seeking potential vendors from all categories: Large and Small Businesses, to include Service-Disabled Veteran Owned Small Business (SDVOSB) and Veteran Owned Small Business (VOSB) contractor(s) for the production and/or supplier of therapeutic shoes. The objective of this information is to provide market research for the availability of Pricing, Data Analysis, and Coding Contractor (PDAC) approved therapeutic footwear medical/surgical supplies for nationwide usage. PDAC is a Medicare contractor responsible for assigning and providing guidance on proper use of Healthcare Common Procedure Coding System (HCPCS) codes. Certain products require code verification review (CVR) in order to be reimbursed by Medicare and most other payers. The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 339113 The Product Service Code is 6515 The Size Standards for NAICS in Number of Employees: 800 Minimum Technical Requirements (MTRs): Offerors must provide matched pairs of production examples (no seconds, salesperson samples, prototypes, etc.) of PDAC approved footwear models currently offered. Samples of women s shoes must be provided in American size/width 9 and 9 1/2 wide, and 10 medium, or equivalent. Samples of men s shoes must be provided in American size 10 ½ medium and 12 medium, or equivalent 1. Has a full length, heel-to-toe filler that when removed provides a minimum of 3/16 of additional depth used to accommodate custom-molded or customized inserts. 2. Is made from leather or other suitable material of equal quality. 3. Has some form of shoe closure. 4. Are shoes available in full and half sizes with a minimum of three (3) widths so that the sole is graded to the size and width of the upper portions of the shoe according to the American Standard Last Sizing Schedule or its equivalent. (The American Last Sizing Schedule is the numerical sizing standard used for shoes in the United States.) All styles being offered shall be available in 75% or more of the sizes in chart listed in appendix D. 5. The offeror will have available for purchase or even exchange inserts meeting the requirements set forth by CMS for A5512 devices that are appropriately sized and shaped to allow for simple exchange of inserts with the ones provided as standard with the shoes without trimming or reshaping (See Note). 6. Therapeutic Footwear shall be available in Men s 8 -14 and Women s 7-11 sizes listed in Therapeutic Footwear Sizes (reference table 1 below). 7. Therapeutic Footwear to include removable inserts at no additional charge. 8. Therapeutic Footwear is modern, has multiple colors options and includes casual, athletic, and business dress: a. Provide details on how often therapeutic footwear catalog is refreshed with new modern designs. 9. PDAC approved Therapeutic Footwear. A PDAC coding decision letter shall be provided for each individual model of therapeutic footwear submitted by the offeror: a. HCPCS Code: A5500 - Description: For diabetics only, fitting (including follow-up), custom preparation and supply of off-the-shelf depth-inlay shoe manufactured to accommodate multi-density insert(s) per shoe. b. HCPCS Code: A5512 - Description: For diabetics only, multiple density inserts, direct formed, molded to foot after external heat source of 230 degrees Fahrenheit or higher, total contact with patient's foot, including arch, base layer minimum of 1/4 inch material of shore a 35 durometer or 3/16-inch material of shore a 40 durometer (or higher), prefabricated. 10. Delivery Times: Orders for therapeutic shoes will be processed and the product shipped within 1 business day. Shipping will be F.O.B Destination and ensures therapeutic footwear can be delivered to all VAMCs within 3 business days following order shipment. Adherence to the order processing and delivery timelines shall be met regardless of the volume of shoes ordered by the ordering VA(s) and is absent of natural or man-made delays. Therapeutic Shoes must meet the following Minimum Technical Requirements (MTRs) for type: TABLE 1 Men s Shoes 1. Men Shoes - Casual, Lace Closure (Black & Brown) 2. Men Shoes - Casual, Strap Closure (Black & Brown) 3. Men Shoes - Athletic, Laced Closure (Black & White) 4. Men Shoes - Athletic, Strap Closure (Black & White) 5. Men Shoes - Business Dress (see Note 1), Laced Closure (Black) 6. Warranty One year exclusive of normal wear and tear Women s Shoes 1. Women Shoes - Casual, Lace Closure (Black & Brown) 2. Women Shoes - Casual, Strap Closure (Black & Brown) 3. Women Shoes - Athletic, Laced Closure (Black & White) 4. Women Shoes - Athletic, Strap Closure (Black & White) 5. Women Shoes - Business Dress (see Note 1), Strap Closure (Black) 6. Warranty One year exclusive of normal wear and tear Note 1: Men s business dress definition - A lace closure men s footwear utilizing shock absorbent, slip resistant material for the sole, an upper made of leather or similar quality material able to be shined with over-the-counter shoe maintenance products intended for this purpose, and the upper styled in a manner suitable for wear in a business or professional occupation setting. Women s business dress definition - A strap closure women s footwear utilizing shock absorbent, slip resistant material for the sole with a heel rise of no more than 1 inch, an upper made of leather or similar quality material able to be shined with over-the-counter shoe maintenance products intended for this purpose, and the upper styled in a manner suitable for wear in a business or professional occupation setting. Therapeutic Shoes must meet the following minimum technical requirements for size: TABLE 2 INDUSTRY FEEDBACK = Request for Information and Comment 1. Do you have an internal quality assurance program which covers the following: How the offeror will ensure on-time delivery How the offeror will respond to carrier issues such as over, short, and damaged How the offeror will ensure the correct product, sizes, styles, color, and inserts are provided to the ordering VA How the offeror will prevent back orders and stock outs 9. Can you support delivery to VA facilities? Additionally, the VA s intent is for a 60-day return policy. Please specify what your company s return policy is. 10. (Optional) Please provide a range from lowest to highest price items. CLOSING STATEMENT: In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the U.S. Government to form a binding contract. Responses to this RFI are strictly voluntary and VA will not pay respondents for information provided in response to this RFI. Responses to this RFI will not be returned and respondents will not be notified of the result of the review. Not responding to this RFI does not preclude participation in any future notices, if any is issued.
