Reference Attachment 1, Amendment dated 13 June 2024 and�Medical Equip Maint Repair and Calibration Solicitation QA 6.13. _________ Amendment 1 : 11 June 2024,��This amendment accomplishes the following: (1) Provided Government responses to questions received from industry (2) Update Solicitation Sections: (a) Performance Work Statement (PWS) and (b) Information to Offerors. All updates within the solicitation are noted in RED lettering/font. Reference Attachment 1, Amendment dated 11 June 2024. __________ Solicitation HT940624R0035�is issued as a Request for proposal (RFP).� The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Regulation as indicated in the attached Standard Form (SF) 1449. The SF1449 form is being utilized to generate the applicable clauses from the Standard Procurement System.� The Defense Health Agency Contracting Activity, Southeastern Healthcare Operations Contracting Division intends to procure a firm fixed price contract for Medical equipment maintenance services in support of Naval Hospital Guantanamo Bay. The on site and off site preventative maintenance, calibrations, and repairs services for the medical equipment must be performed by original equipment manufacturer certified personnel at Guantanamo Bay, Cuba. Contractors must be able to gain base access withing 60 days after contract award. The contract will consist of a base year and four Option years of 12 months each. This requirement is being solicited in accordance with Federal Acquisition Regulation Part 13 � Simplified Acquisition Procedures.� Competition Approach: .Full and Open Competition Submission of Proposals:�Contractors must follow�the requirements of FAR �52.212-1 Addendum, 52.212-2 Addendum, and the Proposal Instructions and Evaluation Criterion provided in the solicitation document to create the proposal. Proposal is required to be submitted electronically to the Contracting Officer and Contract Specialists on or before the date and time specified in SF 1449, Block 8 of Page l. Period of Performance:�The estimated start date is July 28, 2024. The period of performance shall be for one (1) Base year of 12 months and four (4) Option years of 12-month each.� Questions or Clarifications:�All questions are to be submitted via email to the Contract Specialist: erin.l.biser.civ@health.mil and Contracting Officer: lenore.y.paseda.civ@health.mil with email subject: Requirement HT940624R0035.
