- REQUEST FOR INFORMATION / SOURCES SOUGHT for KAVO Focus IO X-ray 85 Reach, Brand-Name or Equal DEPARTMENT OF VETERANS AFFAIRS, VISN 23 Source Sought: The VA is seeking business sources qualified to provide some, or all, of the items listed below as Brand Name or Equal. This is NOT a solicitation. INTERESTED SOURCES SHALL RESPOND BY: Please submit response no later than 10:00am CST, Monday June 24, 2024 All responses and questions must be emailed to Amanda.Patterson-Elliott@va.gov and must include in the subject line: Sources Sought KAVO X-Ray 36C26324Q0893 POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1. Company name, address, phone number, primary contact(s), e-mail address. 2. State if your firm distributes the Brand Name items, or Equal items. 3. SAM UEI number. 4. NAICS (North American Industry Classification System) code that your firm uses for surgical and medical equipment manufacturing. 5. State if your firm is a large business, small business, service-disabled-veteran-owned small business, veteran-owned small business, HUB zone 8(a), women-owned small business or a small disadvantaged business. 6. State if your firm has the required items on Schedule (FSS, GSA, SEWP, other ). 7. Provide a letter from the manufacturer verifying your firm is an authorized distributor. 8. Provide the country of origin for each of the required items. (30 kg) Beam limiting cone dimensions: Round diameter 2.36 inches (60 mm), Rectangular 1.38 x 1.77 inches (35 x 45 mm) SSD (source-skin distance): Standard cone 9 inches (229 mm), Long cone 12 inches (305 mm) Expandability: Remote exposure button, Two remote controls, Door switch readiness, External exposure lamp readiness. Unique characteristics that limit availability to only Brand Name, with the reason no other supplies or services can be used: This requirement is for replacement of current dental Xray units. Iowa City VA currently has Kavo dental x-ray units located within the dental clinic, therefore, no additional plumbing for installation of new dental x-ray arms will be needed. This will avoid extra incurred costs and delay in patient care.
