SCOPE:� Vendor will provides food/misc. items 4 (four) to 5 (five) times per month to the Blackfeet Service Unit. Vendor provides specific foods and beverage to patient diet modifications per physician diet orders. APPLICABLE DOCUMENTS:� Includes established industry standards, methodology and instruments, manuals, precedents in work, literature, established guidelines, regulations, policies, and agency practices, from the Academy of Nutrition and Dietetics Nutrition Care Manual, the Dietary Reference Intakes of the Food and Nutrition Board of the National Research Council of the National Academy of Sciences, and The Joint Commission (PC.02.02.03, EP 7). TASKS:� Establish a Purchase Order for the routine purchase of Food Items for the Blackfeet Hospital Service Unit (BFSU), Browning, Montana, on an �as or when needed� basis in accordance with the following requirements � � � � � The Contractor shall provide: Ordering Fulfill orders placed by the BFSU for items listed, but not limited to, the attached Excel spreadsheet in a timely manner during the period of performance. The Government is obligated only to the extent of authorized purchases actually made under this Purchase Order. The frequency of orders is anticipated to be four (4) or five (5) times per month. Purchase Limitation This purchase order will be a �Not to Exceed� in an amount established at the time of award. Requirements All food items shall be of good quality and procured from approved vendors in accordance with Federal, State, and Administration & Consultant Dietician guidelines. DELIVERABLES AND DELIVERY SCHEDULE: The Contract shall provide weekly deliveries of food supplies to Food Service Department.�� PLACE OF PERFORMANCE: Blackfeet Service Unit (BFSU), 760 Hospital Circle, Browning Montana 59417. Quotes will only be accepted via electronic mail and must be emailed to the CO at enter email address by the closing date. 52.212-2 -- Evaluation -- Commercial Items (Nov 2021) Addenda - Evaluation (a) The Government will award a firm fixed price purchase order (PO) resulting from this solicitation to the responsible quoter whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes:� � � � �� (i).��� Technical. The contractor will be evaluated based upon the capabilities of the contractor as they relate to this requirement as well as their understanding of this project. (ii).�� Past Performance. The contractor will be evaluated based upon their past performance records for Relevance & Quality of past contracts/jobs to determine Performance Confidence, the probability of the contractor meeting the requirements of the project. (iii).� Price. Price will be evaluated for price reasonableness. The attached List of Items will be quoted and the quoted prices are used for evaluation purposes only. All evaluation factors other than cost or price, when combined, are approximately equal to cost or price. Best Value-Trade Offs: The Government reserves the right to make an award to other than the low priced quoter if another superior technical submission, or a submission indicating a reduced performance risk, warrants paying a premium. As non-price factors are evaluated closer to one another between quotes, price will become more significant. Furthermore, the Government reserves the right to award no order at all, depending on the quality of the quotes, the availability of funding and the continued existence of the requirement.
