- COMBINED SYNOPSIS/SOLICIATION:� RFQ FA487724QA368 � FY24 SPORTS RECOVERY KIT i. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and FAR 13 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. ii. Solicitation FA487724QA368 is issued as a Request for Quotation (RFQ). iii. This solicitation document incorporates provisions and clauses of those in effect through, Federal Acquisition Circular (FAC) 2024-05 effective 22 May 2024, Defense Acquisition Circular, (DAC) provisions and clauses effective 30 May 24, and Department of the Air Force Acquisition Circular (DAFFARS) 2024-0612 effective 12 Jun 2024. iv. This is a 100% Total Small Business set-aside. The associated North American Industrial Classification System (NAICS) code for this procurement is 339920 with a small business size standard of 750 employees. v. Davis Monthan Air Force Base intends to award a firm-fixed price contract for the following: �Product Description� CLIN 0001 � Heat and Massage Therapy Wrap � QTY 65: � Unit of Measure: EA� CLIN 0002 � Hand Held Massager � QTY 65 - Unit of Measure: EA CLIN 0003 � Leg Compression Massage Garment, QTY 65 � Unit of Measure EA (See Attached Salient Characteristics for Brand Name or Equal) vi. The government intends to place an order with the offeror whose quote meets lowest price and technical acceptability as defined and referred to within this solicitation document as the offeror�s capability statement to meet the defined salient characteristics of these products. �The Government does retain the right to make multiple or no award in the event it is advantageous to the Government to do so. vii. Important Dates/Times (All times are Arizona Local Time) All questions must be submitted by 10 July 2024 12:00PM (AZ time) All quotes must be submitted via email by 18 July 2024 12:00PM (AZ time) viii. It is the responsibility of the offeror to review the posting for the any changes or amendments that may occur concerning this RFQ.� It is the responsibility of the offeror to ensure all quotes and/or questions are submitted in a timely manner by specified due date/time. ix. Attachment 2 � Provisions and Clauses 5352.201-9101 Ombudsman OMBUDSMAN (JUL 2023) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions). (d) The ombudsman has no authority to render a decision that binds the agency.
