- MILLING AND PATCHING ROADWAYS-BATH AND CANANDAIGUA PROJECT WORK SCOPE DESCRIPTION Existing Conditions: The Canandaigua Veterans Affairs Medical Center (VAMC) is located at 400 Fort Hill Avenue, Canandaigua NY 14424, and the Bath Veterans Affairs Medical Center (VAMC) is located at 76 Veterans Avenue, Bath, NY 14810. Both campuses have a series of asphalt roads, parking areas and pedestrian paths that are in various conditions and require repair. Scope of Work: Contractor is to provide pavement maintenance services as follows: Mill existing asphalt surface to a depth of 2 inches. Material generated by milling activities to be turned over to the VA at a location directed by the Contract Officers Representative (COR). Placement of 2 of 12.5 F3 Top Course Asphalt with series 80 compaction. Locations (to be confirmed with COR prior to starting work): Canandaigua Station: 238 x 31 (7,380SF) at the rear entrance 360 x 24 (13,500SF) on the loop road near Building 4 71 x 9 (639 SF) patch on loop road near Building 37 15 x 12 (91 SF) radius on lot 7 exit Bath Station: 70 x4 (280SF) at the entrance apron to Parking Lot 4. 265 X 9 (2,235SF) pedestrian path by tennis courts. Building 92 rear access drive (5,250SF) and parking area (7,000SF). 10 x 55 (550SF) patch near Building 16 60 x 22 (1,320SF) patch in lot 12 near Building 14 Specifications: Methods and materials for placement of the asphalt shall comply with specifications contained in Appendix A (BATH AND CANANDAIGUA- PM PROJECT). General Requirements: The contractor shall review the contract documents for conflicts. Where conflicts exist work shall not proceed until the COR has been notified and has given direction. Where incidental work is required to accomplish work shown on the drawings, this work is shall be included as part of the base bid. Prior to work commencing in an area, the contractor shall notify the COR of his intention to work in the area and shall schedule a walkthrough with the COR to determine existing conditions. After the work, the existing work areas shall be returned to their original state before final approval shall be given. After the completion of construction, the contractor shall completely clean all surfaces of construction debris. The contractor shall receive all deliveries of materials, equipment, tools, or other goods off station, and bring them to the job site. The Canandaigua VAMC and Bath VAMC shall not accept deliveries for project purposes. The contractor shall not proceed with any on-site work without approval to proceed from the Contract Officer (CO) and COR. Not less than 14 calendar days shall be planned for COR review of each submittal. Re-submittals shall be provided as required by the COR, and shall be treated as a new, separate submittal. Any non-lead time materials will only be paid for when installed. Any items identified as punch list items will be fixed prior to project closeout. Special care will be taken to secure tools and construction equipment during construction as this facility has patients in it. Work cleanliness must be adhered to throughout the project. Sounds and vibrations must be kept to a minimum to avoid patient disruption. The COR reserves the right to stop work if sound and vibrations are impacting patient living or employees. Off tour work may be required to minimize impact to Veterans and employees. The contractor shall warranty all work including for 1 year from the date of acceptance. Warranty shall include any labor or parts necessary to ensure complete functionality of the new area. Submittals: Submittals are not required for this project. Materials are to be consistent with the information listed on the submitted quote forms. Project Duration: The total project duration for this project is not to exceed 60 days. This includes all processes starting with contract award and ending with final punch list completion. Security: The C&A requirements do not apply, and that a Security Accreditation Package is not required. However, vendor must check in with COR upon arrival at Canandaigua and Bath VAMC s at the start of each workday before work commences.
