Description
THIS IS A BRAND NAME OR EQUAL SOURCES SOUGHT NOTICE ONLY. This is not a solicitation for bids, proposals, proposal abstracts, or quotations. The purpose of this Sources Sought Notice is for market research to obtain information regarding the potential availability and capability of qualified Service Disabled Veteran Owned Small Businesses (SDVOSB), Veteran Owned Small Businesses (VOSB), Small Business Manufacturers, other Small Businesses interested and capable of providing items requested, as well as any large businesses. This source sought is UNRESTRICTED and seeks providers capable of fulfilling the requirement in accordance with the Statement of Work. The responses received from interested contractors will assist the Government in determining the appropriate set-aside and the appropriate acquisition method. The intended contract is a firm-fixed price supply purchase. The SBA Non-Manufacturer Rule is applicable under NAICS code 339112 -Surgical and Medical Instrument Manufacturing under which this requirement will be procured. The Department of Veterans Affairs (VA), Network Contracting Office (NCO) 20, Puget Sound VA Health Care System (VAHCS) is conducting market research to identify potential contractor sources which can provide Brand Name or Equal for the following: Specialty Hospital Bed Rental on a Base + 4 Option Years contract for the Department of Veterans Affairs VISN 20 in accordance with the attached Statement of Work. Potential sources having the capabilities necessary to provide the above commodities which meet all the requirements outlined in the attached statement of work are invited to respond to this Sources Sought Notice via e-mail to Phillip.Folger@VA.Gov no later than noted in the RFI. No telephone inquiries will be accepted. Appropriate responses shall include the following information: Company name, address, and business size; point of contact name, phone number, and e-mail address; A capability statement addressing the requirements to include: References for similar commodities currently being provided or previously provided (if any) at the required service levels as outlined in the Statement of Work Capability Statement Capability to provide required supplies at VISN 20 in accordance with the Statement of Work NAICS Code 339112 is applicable to determine business size standard. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. The Government will treat any information received as proprietary and will not share such information with other companies. Any organization responding to this Sources Sought Notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to supply the required commodities. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. The Government may or may not issue a solicitation as a result of this announcement. There is no solicitation available at this time. The Department of Veterans Affairs, NCO20 is looking for BRAND NAME or EQUAL sources of the following (Base+4 OYs): Items Envella® Air Fluidized Therapy Beds Compella Bariatric Beds Statement of Work 1. Scope of Work: The contractor shall provide rental for various type of hospital bed and ancillary equipment to the Seattle and Tacoma VA Puget Sound Health Care System for the sole purpose of patient care for residents and within defined time frames for services. 2. Performance Period: This will be a bulk funded firm-fixed price contract. The period of performance will be for base plus 4 years. Dates to be specified at award. 3. Rental equipment Specifications: We are seeking hospital bed rentals and rentals of other items on the vendor VA FSS schedule. The following items will be delivered as needed to the VA Puget Sound Health Care System. (These are the Brand Name Hill-Rom, but equal items will be considered). If providing an equal product, please ensure enough literature is provided to ensure technical evaluation of equality: Hospital Bed Salient Characteristics: All beds must have labels affixed to the frame to identify the weight limit of the bed and that the bed meets IEC 60601-2-38 specifications. All beds must meet FDA Entrapment guidelines for all 7 zones. All bed controls: Bed is to have separate staff controls on footboard; Staff control must have illuminated buttons and upward angle for ease of use; Hand pendant is also to be illuminated for ease of use; Under bed light is also required to help reduce patient falls; AC receptacles are required at the end of the bed to plug in accessories (will prevent falls) Compella Bariatric Bed: -Powered transport drive. -Powered bed width expansion to reduce effort in bed adjustment. -Powered or a quick and easy length of bed adjustment. -Lateral rotation and low air loss mattress replacement -Patient helper trapeze -Bed low position (sleep surface to the floor) must be no higher than 19 inches. --Bed Hi position (sleep surface to the floor) should have a height no higher than 27 inches to the floor. -Bed dimensions: L 91 X W 43 these dimensions can be expanded depending on patient. -up to 1,000lb weight capacity Envella Air Fluidized Therapy Bed -Must provide pressure redistribution via a fluid like medium; -Must be able to be used to prevent/treat stage I, II and complicated stage III/IV pressure injury, post-operative flap, grafts and burns; -On/Off control for fluidation; -Patient lock out control; -Side rails (full length or quad rails); -Length: 84 to 93 ; Width: 36 to 42 ; Height: min 21.5 ; -Temp control range 86 degrees to 100 degrees -Minimum length and width of bead section 19 W and 45 L -Up to 350 lb weight capacity All bariatric equipment must be inspected by Biomed personnel prior to placing in service. Beds must be FDA compliance and meet the Department of Veterans Affairs Safe Patient Handling requirement. All bed frames greater than 80 in length must have equivalent size mattresses. Contractor to remove trash and all debris from installation and packing materials. Operator Manuals to be provided to users and Service maintenance to be provided as needed on rental equipment. 4. Specific Mandatory Tasks and Associated Deliverables N/A 5. Maintenance: Vendor will provide manufacturer s cleaning instructions and insure all rental property is cleaned and decontaminated prior to bringing to VAMC and must be tagged with name of Vendor for ownership/identification purposes. 6. Service REQUIREMENTS: Operator Training to be conducted to staff on proper use of equipment upon request. In addition: -Vendor is to provide 24/7 Service 365 days a year. -All requests for Bed Frame service are to have a maximum 2-4 hour field response to the facility. -Vendor is to provide a loaner product if a particular frame is to be down for more than 2-3 hours -Bed Frame vendor must have the ability to provide 24/7- 365 days year rental for the above specified frame. 7. Hours of Work: Beds will be delivered on an as needed basis for patients. The contractor shall report to the administrative office of the VAMC to get further directions regarding what room the equipment will be delivered to. 8. Delivery Location: Bed rental deliveries will be to two different locations: a. VA Puget Sound Health Care System, Seattle Division: 1660 S. Columbian Way, Seattle, WA 98108. b. VA Puget Sound Health Care System, American Lake Division: 9600 Veterans Drive, Lakewood, WA 98493. 9. Schedule for returns: Equipment to be returned will be picked-up in 24 hours unless a mutual agreement to hold in event unusual circumstances and delivery will be within same day if request is made by 12 noon. If the request is made after 12 noon, delivery is expected by 12 noon of the following day. Broken equipment must be picked-up and replaced as soon as possible. 10. Identification/Parking The Contractor s employee(s) shall wear visible identification at all times while on the premises of the VA Medical Center. It is the responsibility of the contractor to park in the appropriate designated parking areas. Information on parking is available from the Police and Security Section. 11. PERMITS AND RESPONSIBILITIES The Contractor shall obtain and maintain all permits, licenses, and appointments required for the execution of work under this contract at no additional cost to the Government. The Contractor shall ensure that all employees receive the mandatory training, if applicable to this contract, as required by OSHA, Joint Commission, etc., and shall comply with all standards and/or requirements. 12. CONTRACTING OFFICER OR FACILITY POC The contracting officer reserves the right to designate representatives to act for him/her in furnishing technical guidance and advice or generally monitor the work to be performed under this contract. The Facility POC for this contract will be: Joseph Ignacio 13. CONDUCT OF PERSONNEL The Contractor shall be responsible for the performance and conduct of its employees. Personnel employed by the Contractor in the performance of this SOW or any representative of the Contractor entering the VA premises shall be subject to inspections by the Government as deemed necessary. The Contractor shall not employ for performance under this SOW any person whose employment would result in a conflict of interest with the Government s standards of conduct. 14. CONTRACTOR PERSONNEL The Contractor is completely responsible for all actions of their employees. All contractor personnel employed in the performance of this contract shall comply with all VA building regulations, policies, and guidelines while on the premises. Contractor personnel shall be free of body odor and wear clean clothing (uniforms as appropriate) while performing duties under this contract. The Contractor shall consider the clinical environment and instruct/train their personnel in applicable safety precautions and special requirements. These requirements may include but are not limited to such conditions as human secretions, blood, barium, etc. Contractor personnel shall take appropriate precautions while performing duties applicable to this contract. 15. CHANGES TO CONTRACT: The Contracting Officer is the only person authorized to modify this contract Potential contractors shall provide, at a minimum, the following information to Phillip.Folger@va.gov. 1) Company name, address, and point of contact, phone number, e-mail address, and SAM UID. 2) Please identify your company s size in comparison to the anticipated North American Industry Classification System (NAICS) code 339112 Surgical and Medical Instrument Manufacturing. To be considered a small business your company must have fewer than 1,000 employees. This notice is to determine the marketplace for this specific requirement. Notice to Potential Offerors: All Offerors who provide goods or services to the United States Federal Government must be registered in the System Award Management (SAM located on the web at www.sam.gov). It is desirable that any Respondent should have completed their business Online Representations and Certifications Application in the System for Award Management (SAM). 4) Any Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov. 5) If not a manufacturer, only authorized representatives/providers/distributors of the manufacturer will be considered. Equipment must be new, no used or refurbished equipment accepted. Please provide a proof of authorized dealer or reseller letter from manufacturer. 6) This is a brand name or equal requirement. As such, the salient characteristics reflect the physical, functional, or performance characteristics that equal products must meet to satisfy the Government s needs. To be considered for any potential solicitation, offers of equal products, including equal products of the brand name manufacturer, must: (a) Meet the salient physical, functional, or performance characteristic specified in this sources sought; clearly identify the item by Brand name, if any and make/model number. (b) Include descriptive literature such as illustrations, drawings, or a clear reference to descriptive data or information available that identifies that the equal item meets or exceeds the salient characteristics required by the Government.
