A.3 STATEMENT OF WORK West Texas VA Health Care System George H. O'Brien Jr. Department of Veterans Affairs Medical Center 300 Veterans Boulevard Big Spring, Texas 79720-5566 519-24-602 Relocate Repair Gas Line Statement of Work Background: The West Texas VA Health Care System (WTVAHCS) located at 300 Veterans Blvd., Big Spring, TX. Currently has a requirement for services to replace the natural gas system and all that it entails throughout the facility. Statement of Work: Contractor shall provide all equipment, labor, material, transportation, and supervision to replace the natural gas system and all related equipment within the facility at 300 Veterans Blvd., Big Spring, TX. The contractor will be responsible for visiting the site and interviewing the engineering personnel responsible for the maintenance of the natural gas system and all related equipment. The contractor will be responsible for selecting a natural gas system that will meet or exceed the requirements of the National Fire Protection Associate (NFPA) standards. The contractor shall submit specifications on all materials and show how each will fit the current location of the old system, work with our current utilities, and meet or exceed the requirements of the facility. All material cut sheets must be submitted to the VA prior to purchasing. A person certified in fire protection systems must certify that the materials installed meet all requirements and standards. All personnel performing repairs must be appropriately certified or observed by appropriately certified personnel. Current system may not meet current code requirements, contractor must ensure that new system meet current code requirements. The VA will consider alternative options for replacing the gas system on the facility. The alternative options will need to be approved by the VA prior to the start of this project. Period of performance: 30 Days after notice to proceed is issued. Scope of Work: Locate and mark ALL gas lines within the construction area. Locate and mark ALL lines or line crossings that will be directly or indirectly in the construction area. This will be completed prior to any digging. Removal of all existing natural gas system components, to include all old lines, fittings, union, joints, values inside the construction area. Inspection and assessment of the natural gas systems where connections will be made. Installation of new plumbing, fittings, and valves to connect to existing supply piping. To include pressure test ports and tracer wire / detector tape as needed. Clean up and inspection of work. Contractor to perform final testing and certification. Tasks and Deliverables: The vendor shall provide the specific services and deliverables described below: The vendor shall provide a detailed work plan and briefing for the VA, which presents the vendor s plan for completing the task order. Contractor will plan and coordinate repairs with the COR to accommodate the VA s needs of the facility. The products and pipe used in this repair will be new and adhere to VA specifications. The contractor will be responsible for all unions, fittings, piping, natural gas valves and pressure testing from point to point of this repair. Meaning anything the contractor installs, not the existing pipe or plumbing they will be connecting to. If natural gas system components do not mount in the same location as the old, the contractor will be responsible for installation of new components to make the proper connections. Any insulation that is typical and or requested by the VA shall be installed; this work shall be included in the contractors bid. The contractor shall completely furnish an equipment package that will meet the previously stated Project scope/Objectives. The package shall include detailed submittals, manufacturer literature, and related information as required by the contract. The equipment and install shall be in accordance with the latest Uniform Building Code (UBC), National Electrical Code (NEC), National Fire Protection Associate (NFPA) standard, Uniform Federal Accessibility Standard (UFAS), VA Standards, and local facility requirements. In the event there is a conflict between any of the standards/codes, the more stringent standard/code shall apply. Work shall be coordinated with the COR. Infection control will be limited as this work is away from patient care areas. Any deviation from construction safety, infection control, or other mandated measures will be cause for construction to terminate until these deficiencies are corrected. The contractor shall be solely responsible for the management of installation, including all labor, equipment, materials, and inspection, to meet requirements of the contract. The VA shall provide the contractor any schematic design (layout) or basic construction drawings they have on file. Contractor superintendent shall be always on station to address any needs the VA may have during the construction period. Contractor shall thoroughly clean the construction site daily with contractor s own equipment, including vacuums, mops, buckets, wash rags, dust mops, etc. Contractor shall adhere to basic VA specifications for all work. Contractor shall reference the VA Technical Information Library (TIL). This shall be a turn-key project. Material purchased by the contractor must meet the specifications and submittals shall be approved prior to purchasing the material. Workmanship and materials shall be warrantied for at least one-year from the date of final acceptance. The contractor shall provide all applicable safety signage and barricades applicable to OSHA and VHA. Contractor shall comply with VHA DIRECTIVE 7715 and all VA Center Policy Memorandums, including but not limited to the following. Protection of patients, visitors, and employees from traumatic injury, as well as occupational and facility associated infections. Compliance with OSHA and State construction safety regulations. Compliance with EPA and State environmental regulations. Ensuring that on-site general and sub-contractor s construction workers have completed the OSHA 10-hour construction worker course. Project Superintendent shall possess an OSHA 30-hour certification. Security: This service does not involve contractor connection of one or more contractor-owned IT devices (such as laptop computer or remote connection from a contractor system) to a VA internal trusted (i.e., non-public network); therefore, Information Technology (IT) Security certification and accreditation (authorization) (C&A) requirements do not apply per VA Handbook 6500.6 The C&A requirements do not apply and that a Security Accreditation is not required. Requirements and Considerations: The Contractor is required to comply with security measures and policies set in place by the VA. The Contractor is responsible for testing and ensuring the repair meets requirement standards set by the VA. The Contractor shall be responsible for replacing any damaged surrounding areas during the removal and installation of the components. The Contractor is responsible for labeling all electrical and fire suppression components. The Contractor shall survey all work areas and report any suspicious material before starting work. Any material the contractor finds suspicious and thought to contain asbestos shall be tested and or removed by the VA. Any material that is adjacent to the work area shall be left in place and undisturbed. If work cannot be conducted without disturbing said material, then it shall be tested or abated by the VA. All contractor personnel must report to the Engineering Office at the Facility prior to and after any work performed and sign out after completion of services. The contractor must furnish to the Chief of Engineering a detailed field report, or equivalent, showing work completed before leaving the Facility. The Contractor shall not perform any service that will result in additional charges without prior approval from the Contracting Officer. The Contractor shall comply with the standards in the Occupational Safety and Health Act (OSHA). The Contractor is responsible for his employees while on Facility grounds including any accidents while performing the service. The contractor will fill out a JSA daily, prior to work. The contractor will conduct a One Call prior to digging. The one call number shall be provided to the COR. Contractor must refer to the master spec in the technical information library (TIL) https://www.cfm.va.gov/til/spec.asp as needed. As-Built Drawings: The contractor shall prepare a sector sketch of the repaired line. This will include the location of the line, unions, joints, connections, termination ends etc. This drawing will be used for future reference as needed. This sketch will be considered basis and can be done using google maps etc. Delivery Location: All work related to this contract shall be accomplished at the WTVAHCS, 300 Veterans Blvd. Big Spring, Texas. Competency of Personnel: Personnel shall be authorized by the contractor to perform the services. All work shall be performed by fully qualified and competent personnel. The government reserves the right to authenticate training requirements, request training certificates and credentials from the contractor at any time during the term of the contract. Badges: All Contractor personnel are required to wear I.D. badges during the entire time he/she is on Government property. I.D. badges must have an identification picture and shall state the name of the individual and the company he/she represents. The Contractor is to coordinate with the COR to obtain the I.D. badge from the Police Service. The Contractor is to provide a list of personnel who shall be performing the service.
