Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The solicitation number for this requirement is 75D301-24-Q-77965_CDC_NASN-SBAS Phase 3 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The purpose of this contract is to build on the work from Contract # 75D30121C12121 for the school-based active surveillance among school children. As such, CDC is proceeding with a sole source award to The National Association of School Nurses (NASN) for Phase 3 of the School-Based Active Surveillance (SBAS). A Firm-Fixed Price purchase order will be issued in writing to the successful offeror.
