Description
Purpose: This RFI is being posted to identify capable and interested companies for the requirement. If you are interested and capable of a contract of this type, please respond to the contract specialist, via email, and provide the following information: Business Name UEI Small Business Size Status Statement of Work: The Contractor shall remove and replace damaged concrete located in the drive lane of the east atrium drop-off in front of the Richard L. Roudebush VA Medical Center. The contractor shall complete all work listed below within 60 calendar days following the Notice to Proceed (NTP). Work shall be completed in a way to minimize disturbance of daily traffic flow at the facility. Work may be conducted during normal business hours. The Contractor shall provide all necessary services (i.e., labor, materials, tools, equipment, and competent supervision) to perform all work as noted below to meet the scope objectives and requirements, and to provide the required outcome in terms of overall quality, timeliness, and documentation. All work performed under this project shall fully adhere to all VA Design and Master Specification criteria including energy conservation requirements, as well as relevant additional standards and criteria outlined in the Project Specifications. The Contractor shall be responsible for field verifying existing conditions prior to bid. General repair plan and details are provided in drawing R1. The Contractor shall be responsible for obtaining and coordinating the services of all third-party subcontractors performing services on-site as part of the contract to accomplish the following: Sawcut, remove and replace an estimated 225 square feet of 2 concrete topping slab. Any unused quantities in this total may be reallocated elsewhere in the drive lane at the discretion of the COR. Remove and replace 2 area drains. Contractor shall furnish and install 2 new drains and grates to match existing. Contractor shall include any repairs to the drain system or drain line to provide a fully functional area drain. Remove an estimated 10 square feet of asphalt around area drain located in A-Lot parking. Remove concrete drainage inlet. Install new subbase and compact. Reinstall existing drainage inlet and grating. Replace asphalt with new concrete. At least one drive lane must remain open at all times throughout the duration of the project. Proper barriers and signage must be utilized to redirect traffic flow throughout the project. A flagger may be implemented to redirect traffic. Measures must be taken to reduce dust and debris during demolition. Any air intakes near project area must be properly covered with HEPA filters prior to construction beginning.
