- QUESTIONS FOR 36C25024Q0631 Is there currently an incumbent for this position, or is this a new solicitation for Hazardous Waste Removal? Earth Smart Environmental is the current contractor performing services via two separate contracts. This solicitation combines sharps and RMW into one contract. Hazardous waste, Pharmaceuticals, and Universal waste are not currently being requested under this contract. How many outpatient clinics will require pick-up services under this solicitation? Main medical facility includes Main Hospital, CARES Tower, Atrium Riser, VA Annex and VA Domicilliary. Sharps pick ups will be from every patient room, clinic, and area that currently have sharps containers located, and those new areas of the hospital that are currently or will be under construction in the future. This is a requirement of the vendor to pick up from each location, EMS will not transport sharps containers to the centralized area unless in urgent circumstances. RMW/PATH/and Trace Chemo pick up are currently limited to the following pick-up points: Animal Research Lab, Morgue, Biohazard Ante Room, 4A Soiled Utility Room and Module J Soiled utility room, but more may be added if needed. The following outpatient clinics will require pick-up services under this solicitation. There is a central collection point for all these locations. 1 Akron VA Clinic (CBOC) 55 West Waterloo Road Akron, Ohio Once per week 2 Akron Infusion Center (CBOC) 95 West Waterloo Road Akron, Ohio Three times a week (M, W, F) 3 Canton VA Clinic (CBOC) 733 Market Avenue South Canton, Ohio Once per week 4 Carl Nunziato VA Clinic (CBOC) 1815 Belmont Avenue Youngstown, Ohio Once per week 5 Cleveland VA Clinic: Euclid (ODC) 7000 Euclid Avenue, Suites 102 and 202 Cleveland, Ohio 102 (Daily M, Tu, W, Th, F) & 202 (biweekly/once every other week) 6 Cleveland VA Clinic Superior (OSC) 8901 Superior Avenue Cleveland, Ohio Biweekly (once every other week) 7 David F. Three times a week (M, W, F) Will hazardous waste need to be picked up daily, or are there specific days designated for these services? Please see above Will security card access be required to enter the listed locations? For all locations at the main medical center a PAC card will need to be obtained per instructions in the SOW. Once PAC card is given then it will be activated to open rooms. Rooms located in pharmacy and police services or other secured areas will not work and you will have to buzz to be let in. At offsite locations, the vendor will communicate that they are there to pick up sharps/regulated medical waste. Note only those that have completed a proper DOT course are allowed to sign any manifests. What is the method of payment for this solicitation? Answer: The contractor who is awarded the solicitation will receive a purchase order number to submit their invoicing to the Tungsten Network. Are the pick-up bins located in centralized areas within the facility, and will the hazardous and sharp waste products be placed in secure, discardable bags? Please see above. Sharps and RMW should be discarded into FDA and DOT approved containers or bags, and then placed into a properly labeled transport container, box or cart for shipment away from the facility. Is there a budget that you would like to adhere to for this solicitation? Answer: It is the contractor s responsibility to provide a pricing quote based on the requirement for evaluation. There will be no budget provided.
