Update 16 July 2024� - Added Vendor Question and Answer document (FA701424QB005 - Question and Answer) ========================================== Update 12 July 2024 - Revised Atch. 2 Pricing Schedule Worksheet document to add CLINs for the prerequisite training, conference registration, and certification exam. ========================================== The purpose of this combined synopsis is to establish a Firm-Fixed Price (FFP) Blanket Purchase Agreement (BPA) with a five-year ordering period.�The required training is a lifestyle medicine training package that includes Lifestyle Medicine Professional certification pre-requisite training credentialed by the American Board of Lifestyle Medicine and the American College of Lifestyle Medicine, Lifestyle Medicine Professional certification exam registration, Lifestyle Medicine Conference registration, and a virtual support workshop, in accordance with the Statement of Work (SOW).
