- PR12571480 � U.S. Embassy Bujumbura Forklift Maintenance Services����������������������������� �������������������������������������������������������������������������������������������������Statement of Work (SOW) for U.S. Embassy Bujumbura BACKGROUND: The U.S. Embassy Bujumbura, Burundi stores and fulfills customer orders from a warehouse facility at our embassy warehouse. Consequently, the embassy uses forklifts to move pallets of inventory for shipping and storage. To ensure that the equipment is available and in good condition, regular maintenance is needed to ensure the equipment can be used for the embassy mission and is safe for employees. �� OBJECTIVES: The objective for this SOW is to develop and execute a forklift maintenance agreement at the U.S. Embassy Bujumbura, Burundi to help create more efficient operations.�� SCOPE OF WORK:� The scope of the work is to complete semi-annual and annual maintenance, and repairs, when necessary, on equipment located at the U.S. Embassy to ensure the proper function of each.� The U.S. Embassy ownsurrently owns two (2) forklifts at this location.� There make and model is as follows: �#1 TOYOTA ELECTRIC FORKLIFT TRUCK. MODEL: 8RH 8BRU23. SERIAL NO.:8BRU23-36768. TYPE: E. VOLTAGE: 36V. MAST: 107/240. BASELEG OPENING: 44.00 in. /118mm. MAX. AMPERE HOURS: 1000 AH. BATTERY TYPE: E0.LOAD CENTER ""B"": 24in./600mm. MAX CAPACITY: 4500 lb. /2041kg. #2 TOYOTA ELECTRIC INDUSTRIAL TRUCK (FORKLIFT). MODEL: RRE200H. NO.:6710819. MANUFACTURED: 2019-12. RATED CAPACITY: 2000 kg. WEIGHT: =+/- 5% WITHOUT: 3497kg. BATTERY WEIGHT MAX: 1030kg. BATTERY VOLTAGE: 48V. SPECIFIC TASKS:� � a. Task 1: Perform regular maintenance on forklifts at the U.S Embassy. Complete the correct semi-annual and annual maintenance of forklifts (varying brands) to ensure the safe operation of each to support the embassy. b. Task 2: Assess repairs needed to forklifts when necessary providing the Embassy leadership with: estimated cost for repair, repair parts needed, time to complete the repair, and the consequences if the repair is not completed. c. Task 3: Schedule and complete necessary repairs (as approved) to the U.S. Embassy forklifts to maintain safety and operational use.��� d. Task 4: Vendor will develop a viable maintenance schedule with the U.S.Embassy after an initial visit to assess the equipment. e. Task 5: Maintain maintenance and repair records for U.S.Embassy equipment. The records will be stored at the U.S.Embassy��� DELIVERABLES AND DELIVERY SCHEDULE: Semi-annual maintenance, annual maintenance, and repairs when needed.� The first maintenance cycle will be completed 30 days after the contract is awarded. GOVERNMENT-FURNISHED EQUIPMENT AND INFORMATION:� The U.S.Embassy will provide the forklifts for vendor to perform maintenance and repairs. The U.S. Embassy will also provide the location to conduct the repairs to equipment. The U.S.Embassy will provide access to the equipment in the CBP facility by escort.� �� PLACE OF PERFORMANCE:� Equipment maintenance and repair will be conducted at the U.S. Embassy Bujumbura, Burundi. U.S. Embassy Bujumbura Burundi PERIOD OF PERFORMANCE:� Base period:� One (1) year (365 days) after award Option period one:����������������� One year SECURITY:� The work will be UNCLASSIFIED. However, a U.S. Embassy escort will be provided to the vendor�s representative for maintenance and repairs. SPECIAL CONSIDERATIONS:� a. All repairs must be approved in writing before any work begins on equipment. b. Maintenance and/or repair visits must be coordinated with the U.S. Embassy a minimum of 72 hours in advance to de-conflict normal operations. c. A manufacturer-certified technician must be used by the vendor for all maintenance and repairs.� d. Once the contract is awarded, the vendor needs to coordinate an initial visit to the U.S. Embassy to review equipment to determine a viable maintenance schedule. e. The quotation should include Defense Base Act Insurance. Notes: When submitting your quote, please refer your quote as follows, Quotation Enclosed, PR12571480.
