- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice.� This announcement constitutes the only solicitation; quotes are being requested in response to this notice and a written solicitation will NOT be issued. This solicitation is being issued as a Request for Quote (RFQ) # 246-24-Q-0172.��Submit only written quotes for this RFQ. This solicitation is 100% BUY INDIAN SET-ASIDE. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2024-05.� The associated NAICS code is 423450. This RFQ contains three (3) Line Items: CLIN 0001 Midmark PowerVac G14 Twin, 14-20 User, 2 Vacuum, 220V, 2x30A (Product # 1163-G14) 0002 Installation 0003 Shipping/Freight Any Offeror submitting �OR EQUAL� items must meet or exceed the specifications above (or attached). �OR EQUAL� items must have supporting documentation (statement of work) or OFFERS MAY NOT BE CONSIDERED. PERIOD OF PERFORMANCE: 30 days from date of award. Vendor Technical Requirements: �The contractor shall provide the following for the Pawnee Indian Health Center Dental Department: 1. PowerVac G14 Twin, 14-20 User, 2 Vacuum, Part# 1163-G14 No. Users: 14- 20 No. Vacuums: 2 Total HP: 6 Vacuum (�Hg): Up to 18 Voltage: 208-230/60Hz Total Amps: 20 x 2 On demand suction Energy Savings Quiet Operation Warranty: 5 yr/10,000 hr full-product; 10-yr/20,000 hr pump warranty. 2. Contractor shall provide installation of the PowerVac and all components. Submit Quotes no later than: 07/24/2024 04:00p.m. CDT� to the Following Point of Contact: �via Email: amber.chavez@ihs.gov. Evaluation:� FAR 52.212-2 Evaluation -- Commercial Items (NOV 2021) (completed as follows): (a) The Government will award a firm fixed price contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Lowest Price Technically Acceptable Evaluation Factors: PRICE Technical Requirements To be considered technically acceptable, items must meet the following: The contractor shall provide warranty: Warranty for 60 months or 10,000 hours full product; 10-yr/20,000 hr pump. The Contractor shall provide installation of the PowerVac and all components.
