- Notice of Intent to Sole Source The Department of Veterans Affairs intends to solicit and negotiate a Sole Source, Firm-Fixed-Price Base Plus four (4) Option Year Service Contract with Siemans Medical Solutions USA, Inc. This procurement is for the service and maintenance of Siemens Magnetom Vida, Siemans MRXperion, Siemens T6 Spect CT. The manufacturer is the sole provider of this proprietary service. The North American Industry Classification (NAICS) code for this acquisition is 811310. This procurement is limited to a single source in accordance with FAR Part 13.106-1(b)(2), no other source can satisfy the Agency s requirement. The anticipated award date: 12/01/2024. This requirement is for: Department of Veterans Affairs VA Hudson Valley Healthcare System Castle Point Campus 41 Castle Point Rd Wappingers Falls, NY 12590 This notice satisfies the requirement at FAR 5.201(b)(1), to publicize the proposed contract action. This notice of intent is not a request for competitive proposals and no solicitation will be made available. Any responses received will only be considered for the purposes of determining the marketplace for future procurements. The Government will not pay for information submitted. This will be the only notice of the Government's intent to solicit, negotiate, and award to one source to satisfy this requirement. STATEMENT OF WORK GENERAL INFORMATION Hudson Valley VA Medical Center is looking to purchase a new Base + 4 option years service contract for Magnetom Vida, MRXperion, & Symbia T6 SPECT CT service and maintenance agreement. 1. Scope: The contractor shall provide all labor, materials, equipment and travel for: A one (1) year service with up to four (4) option years on one (1) Magnetom Vida, (1) MRXperion. & (1) Siemens Symbia T6 SPECT CT and the Symbia.Net Single-user All Original Equipment Manufacturer (OEM) required preventative maintenance. On-site repair within 4 hours of a service call Telephone technical support within thirty (30) minutes Parts coverage and parts delivery same day if ordered prior to noon Safety checks and quality assurance Remote service support, device updates, Fixed FD coverage, Tube coverage and remote diagnostics to the site and the equipment 2. Service tags: All equipment receiving routine scheduled service shall be identified by the technician placing an inspection sticker containing the date of service, date of next servicing, and initials of repair technician who serviced the equipment. 3. Service Reports: All maintenance activities need to be documented with a detailed report providing at minimum: identifying the piece of equipment, the maintenance performed, any results, identification of test equipment used, a copy of the test equipment s calibration certificate, condition of equipment and any recommendations for future maintenance or replacement suggestions. Duplicate copies maintained in our records for customer back up. 4. Inspection: If services are required for equipment or a performed task, then an inspection shall be done by the VA Biomedical engineering POC upon completion. The biomedical engineer will perform the inspection in the presence of the contractor personnel, prior to the contractor personnel leaving the facility. No invoices will be processed for payment prior to the completion of POC inspection. If the service is routine and/or performance based, then inspections may not occur for every service date but may occur periodically. 5. Equipment: The service agreement will cover the following government owned equipment: QTY 1 Siemens Magnetom Vida, QTY 1 Siemens MRXperion QTY 1 Siemens Symbia T6 SPECT CT and the Symbia.Net Single-user 6. Performance Period: The period of performance is 12/1/2024 - 11/30/2025 with (4) option years 7. Type of Contract: Firm-Fixed-Price 8. Place of Performance: Castle Point Campus at the VA Hudson Valley Healthcare System. Building F Rm. 003 41 Castle Point Rd Wappingers Falls, NY 12590 9. General Requirements: The Contractor shall comply with Federal, State, and Local Laws, plus any Federal Regulations as applicable to the performance of this contract. The Contractor shall not accept any instructions issued by any person employed by the U.S. Government, other than the Contracting Officer (CO), or the Contracting Officers Representative (COR), all acting within the limits of their authority. The Contractor shall be qualified / authorized by the Original Equipment Manufacturer (OEM) in the repair of all equipment listed within this contract. Contractor shall maintain qualifications throughout entire contract period. Qualification includes, but not limited to, the certification of all repair persons and repair facilities by the OEM as being trained / qualified to perform required repairs. The Government shall maintain the right to seek proof of qualification prior to award and anytime during contract period of performance. Normal hours of coverage are Monday through Friday from 8:00 a.m. to 6:00 p.m., excluding federal holidays. All service/repairs will be performed during normal hours of coverage unless requested or approved by COR or his designee. Federal holidays observed by the Syracuse VA Medical Center are New Year s Day, Martin Luther King Day, Presidents Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. In the event of Primary level service support, the following is required: vendor must be on- site for response within 4 hours, next day parts delivery, unlimited parts coverage, technical phone support, remote services, service management reports, continuing education units, and a 97% uptime guarantee. Downtime will be computed from notification of problem during normal work hours. Scheduled maintenance will be excluded from downtime. Normal work hours are 8:00 a.m. to 6:00 p.m., Monday thru Friday, excluding federal holidays. Operational Uptime will be computed during a month-long time period. Repeated failure to meet this requirement can subject the contractor to DEFAULT action. In the event of Support level service support, the following is required after Biomedical Engineering provides the first response: vendor must be on-site for response within 4 hours, next day parts delivery, unlimited parts coverage, technical phone support, remote services, service management reports, and continuing education units. systems and controls, to ascertain whether the contractor is complying with the information security, network or system requirements mandated in the agreement between VA and the contractor. Any audits or tests of the contractor or third-party designees/partner VA elects to carry out will commence within 30 business days of VA notification. Such audits, tests and assessments may include the following: (a): security/penetration tests which both sides agree will not unduly impact contractor operations; (b): interviews with pertinent stakeholders and practitioners; (c): document review; and (d): technical inspections of networks and systems the contractor uses to destroy, maintain, receive, retain, or use VA information. As part of these audits, tests and assessments, the contractor shall provide all information requested by VA. This information includes, but is not limited to, the following: equipment lists, network or infrastructure diagrams, relevant policy documents, system logs or details on information systems accessing, transporting, or processing VA data. The contractor and at its own expense, shall comply with any recommendations resulting from VA audits, inspections and tests. VA further retains the right to view any related security reports the contractor has generated as part of its own security assessment. The contractor shall also notify VA of the existence of any such security reports or other related assessments, upon completion and validation. VA appointed auditors or other government agency partners may be granted access to such documentation on a need-to-know basis and coordinated through the COR/CO. The contractor shall comply with recommendations which result from these regulatory assessments on the part of VA regulators and associated government agency partners.
