Description

The Indian Health Service, Shiprock Service Unit, Northern Navajo Medical Center, located at US Hwy 491 North, Shiprock, New Mexico 87420 has a requirement to conduct survey data for Ambulatory Care and Emergency Medicine for the Hospital Consumer Assessment of Healthcare Providers and systems. Services must be performed in accordance with the attached SOW. Period of Performace: Base Period of Performance: 09/16/2024 to 09/15/2025 Option 1 Period of Performance: 09/16/2025 to 09/15/2026 Option 2 Period of Performance: 09/16/2026 to 09/15/2027 Option 3 Period of Performance: 09/16/2027 to 09/15/2028 Option 4 Period of Performance: 09/16/2028 to 09/15/2029 The North American Industry Classification System or NAICS code is 541910 � Marketing Research and Public Opinion Polling with a small business size standard of $41.5M The Request for Quotations No. IHS1495591 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be used. Evaluation factors may include technical capability to meet the government need outlined in the statement of work, past performance, and price. Quotes will be due on or by September 10, 2024.
