Description
Please note this solicitation is not ready to be issued as of this date. The Embassy of the United States in Armenia will launch the solicitation on the SAM.gov when it is available. The Embassy of the United States in Yerevan, Armenia will launch, via the Internet, a solicitation to require Health Insurance coverage for its local employed staff and eligible family members, in Embassy Yerevan. The Contractor shall provide Health Insurance services to an estimated 920 local employees, and eligible family members. The American Employees Community Association (AECA) employees, Official Residence Expense (ORE) staff and Peace Corps personal services contractors are included under this contract only as a rider. The Contractor shall furnish managerial, administrative, and direct labor personnel to accomplish all work as required in the contract. These requirements may change depending on funding availability in forthcoming periods. Health benefits include Hospitalization and Surgery, Doctor Visits, Treatments, Medical Exams, Out-patient Services, and Related Diagnostic Services, Prescription drugs, Medical Care Related to Childbirth, Optical Services, Physical Therapy, Dental Services. All responsible sources may submit an offer, which shall be considered. The solicitation will be issued via SAM.gov and the Embassy webpage. All prospective offerors are encouraged to register at SAM (System for Award Management) site prior to the submittal of quotations/proposals. The anticipated performance period is for a base year, with four one-year option periods, if exercised by the Government. All responsible sources may submit a quotation, which will be considered. The Government may award the contract based on the initial offer without discussion. The resultant contract will be a firm fixed price type contract. All contractors shall be registered in the SAM (System for Award Management) Database https://www.sam.gov prior to submitting an offer pursuant to FAR provision 52.204-7. Therefore, prospective offerors are encouraged to register prior to the submittal of quotations/proposals. This opportunity is subject to 52.229-12 - Tax on Certain Foreign Procurements Electronic Submission/Responses: This opportunity allows for electronic responses. Please submit your offer prior to the response date and time. It is important to make sure the submission is made in specific size and format; in MS-Word 2007/2010 or MS-Excel 2007/2010 or Adobe Acrobat (pdf) file format. The file size must not exceed 30MB. �If the file size should exceed the 30MB, the submission must be made in separate files and attached to separate emails with less than 30MB each.
