Combined Synopsis/Solicitation Notice Combined Synopsis/Solicitation Notice Request for Quote: 36C25624Q1487 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, in conjunction with FAR Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Requirement is being evaluated under the procedures of FAR Part 13. As such, in accordance with FAR 13.106-2(b)(3), the quotes will undergo a comparative analysis evaluation to determine which vendor provides the best value to the government in terms of technical capability and past performance, while also providing a competitive price. 2. The combined synopsis/solicitation number is 36C25624Q1487. It is issued as a Request for Quote (RFQ). The Government anticipates awarding one Firm-Fixed Price (FFP) contract resulting from this solicitation. 3. Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2024-06 (eff. 08/29/2024), VAAR Update 2024-10 (eff. 04/09/2024) 4. This requirement is issued as Full and Open Competition. The North American Industry Classification System number is 339114 with a small business size standard of 750 employees, 5. The Department of Veterans Affairs (VA), Michael E. DeBakey VA Medical Center (VAMC) located at 2002 Holcombe BLVD, Houston, TX 77030 is requesting Chairside Economical Restoration of Esthetic Ceramics (CEREC) Primescan AC TP with Steel Sleeve and Sustainment Agreement. The contract shall be for a Firm-Fixed Price. 6. Quotes are due no later than September 23, 2024, at 11:59P.M. Central Standard Time and shall be submitted electronically via e-mail to the Contracting Officer, Jeneice Matthews at jeneice.matthews@va.gov 8. A complete description of the requirement and/or applicable government need(s) are as follows: See attached 36C25624Q1487. Please insert 36C25624Q1487 Quote in the subject line of your email. OFFEROR SHALL COMPLETE THE FOLLOWING LOCATED IN SECTIONS BELOW OF THIS SOLICITATION 36C25624Q1487 TO BE DEEMED ACCEPTABLE: Section B- Contract Administration Data, Contractor contact information. Section B- Price/Cost Schedule, unit price, amount, and grand total. Price Volume. Section E- 52.204-24 Representation Regarding Certain Telecommunications and Video Surveillance Services or Equipment Section E-52.212-3 Offeror Representations and Certifications-Commercial Products and Commercial Services 52.212-3(b)(2) (insert NONE in this fill-in if there are no exceptions), and 52.212-3(v)(2). Responses to this solicitation shall be submitted in two (2) volumes and one (1) administrative addendum/supplement as annotated below: Volume 1 (Technical Volume). Volume 1 shall not contain any pricing information and shall clearly be labeled Volume 1 (Technical Volume). Volume 2 (Price Volume). Volume 2 shall clearly be labeled Volume 2 (Price Volume) and shall not contain any technical information that should otherwise be contained within Volume 1. Addendum/Supplement: Shall contain contractor administrative data and related supporting documents (Section E). EVALUATION: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.
