- (i) This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a separate written solicitation will not be issued. FAR Part 15 combined with FAR Part 12, Subpart 12.6. (ii) Solicitation number 75H70425R00003 applies, and is issued as a Request for Proposal (RFP). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2025-01. (iv) This requirement is a total Total Small Business set-aside and the associated NAICS is 541990 � All Other Professional, Scientific and Technical Services, with a small business size standard of $19.5M. (v) A Firm Fixed Price Purchase Order is expected to be awarded as a result of this RFP. The contract award will include a one hundred and eighty (180) day Base Period. Vendors submitting a response are to provide a fixed price proposal for the Base Period (see Pricing Sheet). Unit price for the CLIN shall include any related materials costs. See the Solicitation Pricing Table for the pricing structure. Please complete and submit the Solicitation Pricing Table as part of any response to the RFP. (vi) The purpose of this fixed price purchase order is to obtain the services of one (1) contractor to provide non-personal professional consulting services in assistance of the Indian Health Service�s Office of Resource Access and Partnerships. This requirement covers the Program Office�s North Dakota & South Dakota Purchased Referred Care Delivery Area expansion feasibility study requirement. See the attached Performance Work Statement for additional details. Remote/off-site performance at the vendor�s facility. Please ignore the location listed, as SAM now requires a City/State entry. (vii) Delivery and acceptance terms for this order, when applicable, is FOB Destination. (viii) The provision at 52.212-1, Instructions to Offerors � Commercial Items (SEP 2023) applies to this acquisition. (ix) 52.212-2, Evaluation -- Commercial Items (NOV 2021) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1) Technical Capability; 2) Relevant Past Performance; and 4) Price. Technical Factors, when combined, are not more important than price. All non-price factors are of equal importance. See the attached Evaluation Factors and Criteria document for details. (b) Options. If applicable, the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer�s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. (d) Comptroller General Examination of Record.
