Description
1. GENERAL REQUIREMENTS The purpose of this contract is to remove trees and grind stumps on the East side of Berlin Dam. All trees shall be cut and removed from project lands within the designated clearing area. All stumps shall be ground to 8� below grade. Designated clearing area is described in the Site Map at the end of this Performance Work Statement. All work shall be conducted in strict accordance with the following standards of practice: ANSI Z133.1 � Tree Care Safety Standard 29 CFR 1910.266 � Logging Operations 29 CFR 1910.269 � Electrical Power Generation, Transmission and Distribution USACE EM 385-1-1 Section 31 Tree Maintenance and Removal 2. GENERAL DESCRIPTION Contractor shall furnish all labor, materials, equipment, tools, transportation, supervision, expertise, and related supplies to safely remove selected trees. 3. SAFETY Hazards may be (but are not limited to) bee stings, snake bites, and overhead electrical hazards. All personal protective gear and safety protocols shall be adhered to by the Contractor and their employees. At any time when the Government representative witnesses a safety violation, work shall immediately be stopped, and the violation documented and corrected before work can resume. All work shall be performed in accordance with the Corps of Engineers Safety Manual EM385-1-1 Latest version is available online at https://www.publications.usace.army.mil/Portals/76/EM%20385-1-1%20_EFFECTIVE%2015March2024.pdf The Contractor shall develop an Activity Hazard Analysis (AHA) for all work under this contract and submit it to the Contracting Officer�s Representative. A blank AHA form is attached (Attachment 1 � Blank AHA Form). The following specific Personal Protective Equipment shall be required under this contract: Safety Shoes � All employees engaged in construction operations shall wear safety shoes meeting the requirements of EM 385-1-1, Section 05.E. Protective Eyewear � All workers on the site shall wear, as a minimum, protective eyewear meeting the requirements of EM 385-1-1, Section 05.A. Additional eye protection may be required in accordance with the requirements of EM 385-1-1, Section 05.B. Hearing Protection � Hearing protection shall be provided for workers in accordance with EM 385-1-1, Section 05.C. Head Protection - Head protection shall be provided for workers in accordance with EM 385-1-1, Section 05.D. Fall Protection - If working from a lift or at heights greater than 6 feet, workers shall use fall protection in accordance with EM 385-1-1, Section 21. The Contractor shall identify and protect all overhead utility lines prior to commencing work in that area. 4. COORDINATION MEETING AND POINTS OF CONTACT (POC) Prior to commencement of any work being performed on site, the contractor shall participate in a meeting with the Berlin Lake Resource Manager and Contracting Officer�s Representative (COR), which will include approving a work progression plan. VERIFICATION OF FIELD CONDITIONS The Contractor shall verify all conditions in the field and shall advise the COR of any discrepancy before performing any work. 6. WORK AREAS The Contractor's attention is directed to the fact that the work to be performed is located in areas heavily used by the public. It shall be the responsibility of the Contractor to provide the controls necessary to prevent any interference by the public to his operations. The Contractor shall take all necessary and reasonable precautions to secure the area against vandalism and to ensure the safety and well-being of the public until final acceptance by the Government. 7. TIME OF PERFORMANCE The Contractor shall confine all operations at the work site between the hours of 7:00 AM and 4:00 PM, Monday through Friday, unless otherwise approved. Work will be permitted on weekends or Federal holidays, with permission of the COR (SCA wage rates will apply). Access guidelines will be provided by the COR. Contractor shall coordinate contract tasks and activities with the COR. 8. SITE VISIT We ask all interested parties please come visit the site on Monday, February 24 at 1:00 PM.
