Description
INTRODUCTION: The Sheridan, Wyoming Veterans Affairs Healthcare System (VAHCS) has a need for a small upgrade project at its Gillette VA Clinic, located at 604 Express Drive, Gillette, Wyoming 82718. This upgrade project addresses existing down spouts and drainage issues; replaces one exterior door and frame; and remodels the casework, finishes, and furnishings in three locations within the Clinic. General: Construction project is to be completed within 45 calendar days from date of Full Notice to Proceed (NTP). Contractor s schedule shall utilize this entire period of performance in their schedule unless otherwise approved in writing by the Contracting Officer s Representative (COR) and Contracting Officer (CO). The CO may issue an Administrative NTP prior to the Full NTP, which may be used for submittal review and materials/supplies ordering lead time. Contractor shall provide all equipment, materials, supplies, tools, software, supervision, and labor required for this project, which may include, but is not limited to, demolition work, providing negative pressure environment for work, and providing all new equipment, utilities, and room finishes as indicated in the plans, specifications, and this Scope of Work (SOW) per VA construction standards. Safety submittals shall be completed and approved prior to the commencement of on-site construction work. Material selection, equipment, casework, and finishes submittals shall be completed and approved prior to ordering of materials for the project. At least one physical sample of all applicable materials/finishes shall be delivered to the VA COR for review and approval. The VA COR is located at the Sheridan VA Medical Center, at 1898 Fort Road (138), Sheridan, WY, 82801. VA will respond to Requests for Information (RFIs) and review submittals within 14 calendar days after receipt from contractor. The building tenants not in the construction areas will remain at work in the clinic during this project. Patients will remain in the clinic in areas not under construction, so the Contractor must make patient safety and infection control measures a priority. The Contractor shall coordinate work locations and schedules with VA COR so as to minimize impact to building tenants. Contractor shall provide a minimum of 48-hours notice to the COR for scheduling their work, so the clinic staff can be informed and notify their patients. All electrical, carpentry, architectural, mechanical work, product design, project management, installation, finishes work, landscaping, and other work needed on this project shall be provided by the Contractor. All required electrical work shall be installed and terminated by the Contractor. No wiring or cabling shall be abandoned in place. All piping, cabling, and wiring shall be labeled. All cabling and patch cabling shall match the Sheridan VAHCS s station-standard colors. Buried utilities must have tracing wire. The installation must, at a minimum, be in accordance with all applicable Department of Veterans Affairs Design Manuals, VA Publications, Master Construction Specifications, Construction Standards, National Fire Protection Association (NFPA), Underwriters Laboratory (UL), International Building Code (IBC), National Electric Code (NEC), all other applicable building codes and standards, and the VA Information Security Handbook 6500.6, 6550.0, etc. The Contractor shall read and be familiar with the most current editions of the applicable codes, standards, guides, and manuals. Any penetrations through any floors, ceilings, or walls shall be fire caulked, or otherwise properly fire sealed, and shall be done in this contract to meet VA standards and NFPA standards. Contractor shall utilize the VAMC s station-standard fire stop products or approved equal product(s). The station-standard products are SpecSeal fire caulk product is SpecSeal SSS Intumescent Firestop Sealant; and, if needed, the SpecSeal Composite Sheet may be used in areas approved by the COR. The Contractor shall inquire of the COR if any existing equipment, fixtures, hardware, doors, frames, or any other property removed is desired to be retained by the VA for reuse elsewhere within the Sheridan VAHCS, at the time of removal. If the VA desires to retain an item, the Contractor shall deliver it to a location designated by the COR. If the VA does not desire to retain an item, the Contractor shall dispose of it properly off-station. The new components installed shall be tested and commissioned, re-certified, and/or tested before VA acceptance. All testing parameters shall be provided by the Contractor with source references to the COR and 3rd-party reviewer before testing begins. Copies of all test reports must be provided to the COR. HVAC systems that have changes made to them as part of this project, beyond diffuser replacement, shall be tested and balanced by the Contractor. Any IT data cables installed or modified as part of this effort shall be tested after installation. Test methods and plans shall be coordinated with the COR. Fire alarm devices that are tied into the building fire alarm system, that are replaced or relocated as a result of this work, shall be tested. The Contractor shall arrange for all required fire alarm testing. If a building s fire alarm system is under warranty, testing must be accomplished by the contractor who holds the warranty. Any fire sprinklers that are relocated as part of this project shall be reviewed and approved by VA Safety prior to installation. All fire sprinkler work shall be coordinated with VA Safety so as to avoid accidentally setting off the system. At the conclusion of the project, the Contractor shall provide the VA with a red-lined set of construction drawings, showing any as-built variations from the plans, as called for in the specifications. The Contractor must repair any damage caused by the Contractor or their subcontractors during this project. Repairs shall be done timely and will be coordinated with the COR. Repairs shall be of equal or better quality and finish than the existing that was damaged. Contractor s work hours shall be between 8:00am-4:30pm Monday-Friday, unless otherwise coordinated with, and approved in advance by, the COR. Noisy work shall not begin prior to 8:00am. Contractors shall not work on weekends, or federal holidays as specified by U.S. Office of Personnel Management (OPM), unless approved in advance by the COR or CO. For approved weekend or holiday work, Contractor must adjust their workers pay accordingly at no additional cost to the VA. (At time of writing, the OPM-specified federal holidays include: New Year s Day, Birthday of Martin Luther King Jr., Washington s Birthday, Memorial Day, Juneteenth National Independence Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day.) The Contractor shall repair any disturbed earth around and in the construction areas. This shall include: Removing any clumps or unwanted rocks that have been brought to the surface. Filling in all ruts or depressions with approved fill material, such that the areas are level or at the required slope. Returning the surface cover to what it was before the project, or as called out in the project drawings. In the case of grass, the contractor shall provide topsoil as necessary to provide a good medium for grass growth. If the area(s) cannot be covered by the VA sprinkler system, then the Contractor shall provide for the watering of the new grass until it is established. Any areas that the grass does not take to shall be reseeded under the warranty period. Excessive weeds, as determined by the COR, will not be accepted. The Contractor shall replace any trees, shrubs, or other plants damaged or destroyed during construction, in kind, at no additional cost to VA. Substitutions must be pre-approved by the COR. Contractor shall obtain and pay for all required federal, state, and/or local permits for this project. Contractor shall obtain utility locates for the project areas before beginning any excavation work. Safety and Security: The Contractor shall follow all safety specifications and shall always have an OSHA-30 certified person on-site, at each work location, overseeing the construction. All other Contractors and subcontractors working on-site shall have at least OSHA-10 certifications. Certification copies shall be provided to the COR prior to beginning work. The Contractor shall submit a Site Safety Plan, tailored for the specific project, to the COR for review and concurrence before beginning work on-site. All soil at the Gillette VA Clinic shall be treated as OSHA Type C soil (i.e., no benching). Contractor is responsible for knowing and complying with OSHA requirements as applicable for the project. The Contractor will review the Infection Control Risk Assessment (ICRA) with VA Infection Prevention & Control staff. The Contractor shall implement all protection measures called for as a result of the risk assessment. Some work will be within the clinic, with staff and patients present, so the Contractor shall plan for infection control measures, to include dust containment, sticky walk-off mats, negative pressure with monitoring, use of High-Efficiency Particulate Air (HEPA) filter vacuums, and cleanup to hospital standards. The Contractor shall read and follow the current version of the FMS Safety Plan, which outlines expectations for tool control and safety within the Sheridan VAHCS. Contractor shall distribute the FMS Safety plan to their workers and subcontractors. Contractor shall note this training in their daily logs. For this project, the Superintendent and Site Safety & Health Officer duties may be performed by the same person. General safety requirements: It is vitally important to keep tools and other items, that offer a patient the opportunity for self-harm, under control at all times. Tools, extension cords, and similar items should not be left out unattended where patients can gain access to them. This includes in the open backs of contractor vehicles, trailers, laydown areas, tool carts, etc. Tools should be locked in a job box, some other type of toolbox, or within a trailer or CONEX box that can be secured from unauthorized access as approved by the COR. Contractors shall check their work areas as the day s work progresses for tools left unattended (e.g., screwdriver rolling under furniture, razor blade left in the grass outside, reels of cords that could hang someone). Large storage containers must be locked if not occupied by a staff member and during overnight. Tools and supplies left out in laydown areas must be monitored, as patients can walk the campus. Also, site cleanliness is an OSHA requirement. Badging requirements: All Contractors and subcontractors must obtain a temporary contractor badge from the VA COR before beginning work on-site. A valid (i.e., not expired), Government-issued, photo ID (such as a Driver s License) is required to be issued a badge. Contractors who have a valid PIV Badge or CAC Badge will not need to be issued a temporary badge. (Due to this contract being at the Gillette VA Clinic, and not the Sheridan VA Medical Center, VA COR will coordinate with Contractors during the pre-construction meeting for how badges can be obtained.) For Contractors on-site less than 179 calendar days per year, the above will suffice, and Contractor PIV badges are not needed. An exception to this: a Contractor PIV badge is needed (regardless of work length) within IT closets and other designated secure areas. (For IT closets specifically, it is permissible for just one Contractor with a PIV badge to supervise the work of their contractors at all times within the IT closets.) For Contractors on-site 180 calendar days or more per year, they are required to obtain Contractor PIV badges, which requires fingerprints and a background check process. Contractor PIV badges are valid for 6 months maximum. It is the Contractors responsibility to ensure their badges stay valid and are renewed timely. Contractor shall check with COR at pre-construction meeting for the most-current badging requirements and for beginning the Contractor PIV badge process, as required. Contractor will be provided space for one Conex/storage container on-site at the Gillette VA Clinic, if necessary, to use during this project. This space will be assigned by the COR or CO after contract award. Phasing There are no prescribed phases for this project. Contractor must submit a proposed schedule for review and approval by both the COR and CO before beginning any work on-site for this project. The Contractor s schedule must allow for 48-hours minimum inspection time by the COR and/or VA for completed work. The Contractor must propose a phasing plan for review and approval by both the COR and CO before beginning any work on-site for this project. The order of work on the different phases shall be coordinated closely with the COR. Contractor shall provide 48-hours minimum advance notice of planned work, so COR can inform the staff and occupants in the area. More than one area may be worked on concurrently. However, Contractor must plan their work to minimize impact to patients and staff to the extent possible, including the duration of work in occupied rooms and patient-care spaces. Contractor shall provide and post the signage necessary to guide patients to alternate check-in/out areas during construction (e.g., when Reception casework is being repaired/replaced). Signage wording shall be approved by the VA COR and/or clinic staff before posting. Contractor must maintain ADA access to the Patient Restrooms (Rooms 135 and 138), throughout the Waiting Room 102, and all Corridors in the clinic throughout construction. If an exit needs to be blocked, the Contractor must notify the COR at least 48 hours in advance, so an Interim Life Safety Measure (ILSM) can be filed, posted, and implemented. The Contractor s schedule must allow for 48-hours minimum inspection (and cleaning) time by the COR and/or VA for completed work. Contractor shall relocate the existing furniture, per the COR s guidance, as needed for the staff to have temporary workstations elsewhere while the casework is being repaired/replaced. (VA IT staff, or Clinic staff, will relocate computer workstations and phones as required.) Work shall include: Base Bid: Contractor s work shall include all items listed herein this SOW and in the drawings, specifications, and details provided, unless items are specifically called out as VA-provided materials or labor. **Contractor shall field verify all quantities, sizes, dimensions, and field conditions and receive approved submittals before ordering or using products and supplies for this project. Alternates: There are no additive/deductive bid alternates for this project. Description of work: The existing downspouts at the Gillette clinic have elbows just above the ground and surface drain without splash blocks into rock planter beds. The clinic has had issues with settling in the planter beds and water infiltrating its basement due to drainage issues. This project shall correct the existing downspouts, so they drain properly away from the building. The west exterior door and frame have settled since the building was constructed in 2009. This has caused intermittent issues with the door operation and its hardware. Contractor shall demolish and replace this existing door and its frame, as well as all associated hardware and accessories needed for a fully-functional, compliant, and safe system. The existing door is insulated hollow-metal and has a powered door operator with ADA push buttons. Contractor shall make the adjustments necessary to ensure the new door and frame are plumb and level. Contractor shall also replace the existing concrete door sill and sill plate. Work shall be according to the drawings, details, and specifications. The existing casework at the Gillette clinic s Reception area ( Room 104 ), HIPPA check-in area ( Room 103 ), and Nurses Station ( Room 107 ) shall be demoed, procured, and replaced per the drawings, details, and specifications for this project. The existing casework is damaged in areas, particularly its laminate finishes. This is an infection prevention and control issue because it cannot be cleaned properly with healthcare-grade cleaners. Also, a few areas may present safety hazards where the laminate is peeling or where the casework has been damaged. These need to be corrected to ensure staff, Veteran, and visitor safety into the future and to extend the life of the casework. Contractor shall install, or relocate existing, power and data outlets for the workstations per the drawings, details, and specifications. Contractor shall demo and replace the finishes (e.g., flooring, paint) as scheduled in the project plans. The Contractor shall provide storage for all materials, equipment, and new furniture until they are installed. The furniture storage must be climate-controlled and meet the manufacturers specifications to protect the furniture and finishes from damage. The existing furnishings within scope that are not scheduled for re-use shall be retained for VA use elsewhere within the clinic or VAHCS. List of VA-Provided Items/Services: The VA will provide and install the desktop sit-to-stands. VA IT will provide and install the workstation computers, phones, and patch cabling from the walls to the workstations. VA will provide and install surge protectors for the workstations if needed. VA COR works in Sheridan, WY but will make periodic trips to Gillette as needed for coordination and quality assurance. (Contractor must be responsible for quality control.)
