- Amendment 6 is posted to Synopsis/Solicitation HT940626QE005 to correct the quantity for the SLIN under CLIN 2002. The Pricing Template has also been updated to add the rows for Option one (CLIN 1001) Please see attachments. Interested contractors must submit their quotes before or on February 2, 2026 not later than 2:00pm Eastern Standard Time as stated in SF1449. If these changes affect the unit price for those contractors that submitted a quote on or before January 20, 2026 at 2:00pm, please submit your revised quote before or on February 2, 2026 not later than 2:00pm Eastern Standard Time as stated in SF1449. Otherwise, please send an email to the Contracting Officer and the Contract Specialist acknowledging the solicitation amendment 6 and stating that the changes do not affect your pricing in the quote submitted on January 20, 2026. ***************** Attached is Q&A document - Questions and Answers - HT940626QE005 16JAN26 - to provide answer to questions from industry. No changes to the solicitation have been made. **************** Amendment 5 is posted to Synopsis/Solicitation HT940626QE005 to (a) update bullet points 1.4.5 (2nd paragraph) and 2.1.10. (b) An updated Q&A document is also attached to provide answers to questions from industry. Please see attachments inlcuding a revised PWS and answers to submitted questions. ******************* Synopsis/Solicitation HT940626QE005 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Regulation as indicated in the attached Standard Form (SF) 1449. The SF1449 form is being utilized to generate the applicable clauses from the Standard Procurement System. The Defense Health Agency Contracting Activity, Southeastern Healthcare Operations Contracting Division intends to procure a firm fixed price contract for water sample collection and analysis services for three (3) locations, Shaw AFB, Poinsett Range, and Wateree Air Force Recreational area. The contractor shall collect, label, transport, and analyze water samples from various sources including surface water bodies (e.g, lakes, rivers), ground water sources (e.g, wells), and points within water distribution systems. The contractor must assess water quality, determine compliance with relevant regulations, and provide recommendations for remediation or treatment, if necessary. The contract will consist of a base year and four Option years of 12 months each. This requirement is being solicited in accordance with Federal Acquisition Regulation Part 13 � Simplified Acquisition Procedures. Submission of Quote: To create the quote, Contractors must follow the Quote Instructions section and the requirements of FAR 52.212-1, FAR 52.212-2, 52.212-1 Addendum, 52.212-2 Addendum included in the solicitation document. Quote is required to be submitted electronically to the Contracting Officer and Contract Specialists on or before the date and time specified in SF 1449, Block 8 of Page l. Competition Approach: Small Business set-aside. Period of Performance: The estimated start date is April 1, 2026. The period of performance shall be for one (1) Base year of 12 months and four (4) Option years of 12-month each. Questions or Clarifications: All questions are to be submitted via email to the Contract Specialist: yuleiny.garciacontreras.civ@health.mil and Contracting Officer: lenore.y.paseda.civ@health.mil with email subject: Requirement HT940626QE005. The Government may not respond to questions received less than 3 days prior to the due date and time for receipt of quotations.
