- The Department of Veterans Affairs (VA), VISN 08, San Juan Department of Veterans Affairs Medical Center (SJVAMC), Veterans Affairs Caribbean Healthcare System (VACHS), 10 Casia Street, San Juan, Puerto Rico 00921, intends to solicit an unrestricted full and open competitive Request for Proposals (RFP), to provide services necessary to perform onsite Cardiothoracic Surgery Physician Services to eligible beneficiaries of the VACHS. The Contractor s physician(s) care shall cover the range of Cardiothoracic Surgery services as would be provided in a state-of-the-art civilian medical treatment facility and the standard of care shall be of a quality, meeting or exceeding currently recognized national standards as established by American Associate for Thoracic Surgery (AATS): https://www.aats.org/, under an Indefinite Delivery Indefinite Quantity (IDIQ) contract, for a base plus four (4) one year option periods. The contractor shall provide Board Certified /Board Eligible Cardiothoracic Surgeon Physician services to deliver onsite Open-Heart Surgery-Cardiothoracic Surgery services, which shall include, but not be limited to, initial patient evaluation in inpatient, outpatient and Emergency Department (ED) settings and surgical procedures in Operating Room and post operative care settings, including tasks related to Cardiothoracic Surgery Consult (operation not recommended), Permanent Pacemaker Implant (in emergency cases), Pacemaker Lead Extraction (back up), Pericardial Window and/or Pericardiectomy Procedures, back up for Interventional Cardiology (PCI) cases, onsite and on-call in accordance with the terms and conditions set forth in the Performance Work Statement. The Contractor shall propose a minimum of two (2) key personnel Cardiothoracic Physician Surgeons to be credentialed and be available for simultaneous scheduling to meet the requirements of the contract. The two (2) key personnel surgeons must begin the onboarding process upon contract award and must both begin providing services on the start date of the Period of Performance of the base year to ensure coverage for all required hours. One (1) physician surgeon is required to be available on-site for scheduled elective procedures One (1) physician surgeon is required to be available on-site for scheduled elective procedures (Operating Room) and patient evaluations at clinics on Mondays and Wednesdays from 7:00am to 3:30pm. One (1) physician surgeon will be on call during weeknights Mondays and Wednesdays from 3:30pm to 7:00am, and weekdays Tuesday, Thursday, Friday, weekends, and holidays from 7:00am 7:00am (24 hour daily coverage) for emergency cases and Cath Lab procedures; as needed, surgeon will be called to be onsite as stand by for specific/elective Cath Lab procedures during the week; in addition, this physician needs to attend scheduled meetings on Friday mornings from 7:30 8:30 am for discussion of cases. Contractor s key personnel must be available on-site within one (1) hour of notification for back-up and/or emergency cases. QUALIFICATIONS: License: The Contractor s physician(s) assigned by the Contractor to perform the services covered by this contract shall have a current license to practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia when services are performed onsite on VA property. All licenses held by the key personnel working on this contract shall be full and unrestricted licenses. Contractor s physician(s) who have current, full and unrestricted licenses in one or more states, but who have, or ever had, a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action or denied upon application will not be considered for the purposes of this contract. Board Certification: All Contractor s physician(s) shall be Board Certified /Board Eligible by the American Board of Thoracic Surgery and be currently certified in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) or equivalency. All continuing education courses required for maintaining certification must be kept up to date at all times. Documentation verifying current certification shall be provided by the Contractor to the VA COR on an annual basis for each year of contract performance. Credentialing and Privileging: Credentialing and privileging is to be done in accordance with the provisions of VHA Directive 1100.20 and VHA Directive 1100.21 referenced above. The Contractor is responsible for ensuring that proposed physician(s) possesses the requisite credentials enabling the granting of privileges. The period of performance is expected to be from July 1, 2026, through June 30, 2031. The authority of for this procurement is Title 38 USC 8153, Health Care Resources (HCR) Sharing Authority, Federal Acquisition Regulation (FAR) Overhaul Part 12 Acquisition of Commercial Products and Commercial Services, and FAR Overhaul 15, Contracting y Negotiation.
