Description

AMENDMENT 002: TO CORRECT/ANSWER AND CLARIFY QUESTIONS DATES: 1. 1.5.6 indicated thirty-minute lucn break, and paragraph 5.2.1 indicaters 1-hour lunch break. It is 1-hour lunch break. 2. Paragraphs 4.1.1 & 4.1.2 incomplete. Leave 4.1.2 as is. 4.1.1 The contractor shall provide all uniforms. 3. 5.1.13 It will be at the discretion of the health center, and can be a all inclusive rate. 4. 5.2.3 On call duties, will be at the discretion of the COR. 5. 5.8.4 & 16.3.3 Health center looks at pricing and LPTA is one they look for as well, along with the best value, and other factors. 6. 9.1 Base Year + 4 Option Periods. 7. 12.5 Removed 8. 16.5.4 These are instructions, put administratiave assistants. 9. 16.6.1 & 16.6.2 Leaving as is. All other terms and conditions shall remain unchanged.
