- The Bay Pines VA Healthcare System, Lee County VA Clinic, at 2489 Diplomat Parkway East, Cape Coral, FL 33909-5422, has a requirement for Medical Gas Education. All vendors must be properly registered in the System Award Management database (SAM) and SBA Dynamic Small Business Search (DSBS). Price quotes shall include the Unique Entity Identifier (UEI) Number, SBA Dynamic Small Business Search (DSBS) SDVOSB certification, any other supporting information for the price submitted, and any items identified in ADDENDUM to FAR 52.212-1 Instructions to Offerors.
