Q -- MUTUAL USE SHARING AGREEMENT FOR PRIMARY CARE COMMUNITY BASED SERVICES SOL RFP 533-2-95 DUE 030695 POC Contact Point, Bertha Geraghty, (914)831-2000x5262, Contracting Officer, FAX 914-831-1634 The Department of Veterans Affairs Medical Center, Castle Point, NY is seeking qualified health care providers capable of delivering appropriate primary care services to Veteran beneficiaries in the Middletown, NY geographic area. It is contemplated that a single award will be made to a facility qualified to provide primary care services such as but not limited to physician, physician assistant and/or nurse practitioner and nursing services support for an estimated volume of primary care clinic visits as well as clinical equipment and supplies, ancillary laboratory tests, routine radiology examinations, pulmonary function tests, EKGs and available pharmacy services to qualified Veterans in accordance with contract terms and conditions for the period of date of award through September 30, 1995 with three (3) one-year government renewal options. The area of consideration for the location of the facility is limited to Middletown, NY and surrounding communities. The estimated date of issuance of the Request for Proposal is January 30, 1995 with an anticipated date for receipt of proposals of March 6, 1995.
